整体护理干预在瘢痕子宫再次妊娠阴道分娩产妇中的应用效果

Effect of Holistic Nursing Intervention on Vaginal Delivery Again for Pregnancy in Scar Uterus

目的探讨整体护理干预在瘢痕子宫再次妊娠阴道分娩产妇中的应用效果。方法选取2012—2014年钦州市第一人民医院收治的瘢痕子宫再次妊娠分娩产妇76例为研究对象,采用随机数字表法分为对照组和观察组,每组38例。对照组产妇给予常规护理方法;观察组产妇给予整体护理干预。比较两组产妇的产前抑郁指数、产前焦虑指数,经阴道分娩成功率、产程时间、产后出血率。同时对两组产妇进行护理满意度调查。结果观察组产妇产前抑郁指数(38.4±3.2)和产前焦虑指数(32.3±6.2)均低于对照组产妇的产前抑郁指数(43.2±4.6)和产前焦虑指数(38.7±6.1),差异有统计学意义(P<0.05);观察组产妇经阴道分娩成功率86.8%(33/38)高于对照组39.5%(15/38),产程时间(11.42±1.33)h少于对照组(16.21±2.42)h,差异有统计学意义(P<0.05)。观察组产妇产后出血率7.9%(3/38),对照组产妇产后出血率18.4%(7/38),差异无统计学意义(P>0.05)。观察组护理满意度优于对照组,差异有统计学意义(P<0.05)。结论整体护理干预在瘢痕子宫再次妊娠阴道分娩产妇中能有效降低产前抑郁、焦虑程度,提高产妇的阴道分娩成功率、缩短产程时间,且能提高护理满意度。

Objective To explore the effect of holistic nursing intervention on vaginal delivery again for pregnancy in scar uterus. Methods Seventy - six women who had undergone pregnancy in scar uterus in the First People＇s Hospital of Qinzhou from 2012 to 2014 were selected as study subjects, were randomly divided into control group and observation group, each group of 38 cases. The control group was given routine nursing, the observation group was given holistic nursing intervention. Maternal prenatal depression index, prenatal anxiety index, success rate of vaginal delivery, labor time, rate of postpartum hemorrhage were compared between two groups and the nursing satisfaction was investigated between two groups. Results The maternal prenatal depression index （38. 4 ±3. 2） and the prenatal anxiety index （32. 3 ±6. 2） in the observation group were lower than those in the control group （43. 2 ±4. 6） （38. 7 ±6. 1）,the differences were statistically significant （P 〈 0. 05）. The success rate of vaginal delivery in the observation group 86. 8% （33/38） was higher than that in the control group 39. 5% （15/38） , labor time in the observation group （11. 42 ± 1. 33 ） hours was lower than that in the control group （16. 21 土 2. 42） hours, the differences were statistically significant （P 〈 0. 05 ）. The rate of postpartum hemorrhage in the observation group was 1.9% （3/38）,in the control group was 18. 4% （7/38）,the difference was not statistically significant （ P 〉 0. 05）. The nursing satisfaction in the observation group was better than that in the control group, the differences were statistically significant （P 〈 0. 05 ）. Conclusion Holistic nursing intervention on vaginal delivery again for pregnancy in scar uterus can reduce the degree of depression, anxiety, improve the vaginal delivery rate of pregnant women, shorten the labor time and can improve the nursing satisfaction.
