医疗机构介入放射工作人员个人剂量及辐射水平调查 被引量:3

The Study of Exposure Dose and Radiation Level for the Interventional Radiology Staff

摘要 目的:了解介入放射工作人员个人接触剂量及其辐射水平,以保护介入放射工作者的身体健康。方法:于2015年随机抽取某市6家开展放射介入学的医疗机构,对131名介入放射工作人员开展1年4期个人放射剂量监测,对10台介入放射设备手术者位和助手位按头部、胸部、腹部、下肢和足部位置进行透视区辐射水平检测。结果:131名介入放射工作人员中男性66.4%(87/131),女性33.6%(44/131)。个人年累积剂量均<5 m Sv,年人均有效剂量中位数0.31 m Sv/a,(Q25,Q75)为(0.22,0.47)m Sv/a。10台介入放射设备,透视防护区工作人员位置辐射剂量大于400μGy/h占19.0%(19/100),其中手术者位20.0%(10/50),助手位18.0%(9/50),两者合格率比较差异无统计学意义(χ~2=0.07,P=0.80)。手术者下肢位辐射剂量最高为459.40μGy/h,其次为腹位215.80μGy/h,手术者位不同部位辐射水平比较差异有统计学意义(χ~2=11.05,P=0.03);助手腹位辐射剂量最高为303.95μGy/h,其次为下肢位257.25μGy/h,助手位不同部位辐射水平差异有统计学意义(χ~2=13.54,P=0.01)。手术者与助手的不同部位辐射水平比较差异无统计学意义(Z=-0.45,P=0.66)。结论:某市介入放射工作人员个人剂量水平较低,透视防护区工作人员腹位、下肢位存在漏射线,应加强辐射防护。 Objective: To study the exposure dose and the radiation level of the interventional radiology staff for protected the health of them.Method: 6 hospital with the interventional radiology were selected in 2015, the exposure dose of 131 staff with interventional radiology were monitored 4 times in 1 year.The levels of radiation in the area of head, chest, abdomen, legs and feet were measured in 10 interventional radiology devices.Result: The 131 staff with interventional radiology had male 66.4% ( 87/131 ), female 33.6% (44/131) .Individual integral doses were 〈5 mSv, the median of annual collective effective dose equivalent was 0.31 mSv/a, the ( Q25, Q75 ) was ( 0.22, 0.47 ) mSv/a.There were 10 digital subtraction angiography (DSA), 19.0% ( 19/100 )zones' irradiation dose where the operators and assistances stood were over 400 μ Gy/ h, irradiation dose of operators' lower limb highest was 459.40 μ Gy/h, there were statistical differences of the irradiation dose levels by the different parts of operator ( x2=11.05, P=0.03 ) .The irradiation dose of assistances' abdomen highest was 303.95 μ Gy/h, there was statistical differences of the irradiation dose levels by the different parts of assistances ( χ2=13.54, P=0.01 ) . But there was no statistical differences of the different parts irradiation doses between operators and assistances by Wilcoxon rank sum test ( Z=-0.45, P=0.66 ) .Conclusion: A city in the radiation workers' individual dose level are low, the staff radiation of abdomen and lower limbs of the protected area have leaking rays should be strengthened.
