支气管热成形术治疗难治性哮喘五例临床观察 被引量:4

Clinical observation of bronchial thermoplasty in the treatment of 5 patients with refractory asthnm

摘要 目的探讨支气管热成形术(BT)治疗难治性哮喘患者的临床疗效。方法回顾性分析接受BT治疗的难治性哮喘患者5例(15例次)的手术情况,比较治疗前、三次手术治疗后3周及第三次手术后12周者临床症状,分析肺功能、呼出气一氧化氮(FeNO)、6min步行距离、哮喘控制测试表(ACT)评分变化。结果5例难治性哮喘患者在病情稳定期于手术室接受BT支气管热成形手术治疗共15例次,手术过程顺利,患者耐受性好,5例次患者术后12h内出现咳嗽加重、痰液黏稠及呼吸困难加重,程度均不重,给予拍背排痰、雾化吸入支气管扩张剂及经支气管镜取出痰痂引流后均明显好转,1例次患者手术后出现手术区域肺炎表现,经给予支气管镜取出痰痂,并抗生素治疗5d后肺炎痊愈;5例患者术后各观察节点呼吸困难程度均较前明显改善,运动耐量提升,FeNO均逐渐下降,ACT评分增加,但肺功能无明显改善;肺部高分辨CT监测提示肺部结构无显著改变。结论BT是对难治性哮喘患者传统药物治疗无效情况下的有效补充治疗手段,能显著改善患者生活质量,患者能从每次手术后受益,同时患者耐受性好;但治疗后患者肺功能无明显改善。
