中药内服联合溻渍法治疗急性肛周湿疹临床观察

Clinical Observation of Traditional Chinese Medicine Oral Administration Combined with Chinese Herb Hydrotherapeutics for Treating Acute Perianal Eczema

目的:观察中药内服联合中药煎汤溻渍疗法治疗湿热型急性肛门湿疹的临床效果。方法:将确诊的180例湿热下注型急性肛门湿疹患者采用完全随机分为3组,中药内服溻渍组60例,中药熏蒸内服组60例,对照组60例,中药内服溻渍组以祛湿止痒外用方溻渍联合龙胆泻肝汤内服治疗,中药熏蒸内服组采用局部祛湿止痒外用方熏蒸联合龙胆泻肝汤加减治疗,对照组采用局部外涂曲安奈德益康唑乳膏治疗,疗程均为10天,并比较3组患者的临床疗效及复发率。结果:治疗后3组患者在肛周瘙痒、肛门潮湿、皮损修复情况等症状缓解。与对照组比较,中药内服溻渍组、中药内服熏蒸组治愈率及复发率均降低(P<0.05),与中药内服熏蒸组比较,中药内服溻渍组复发率较低(P<0.05),差异有统计学意义。结论:祛湿止痒外用方溻渍联合龙胆泻肝汤加减内服治疗湿热型急性肛门湿疹安全、有效,在改善患者肛周瘙痒、肛门潮湿、皮损修复以及减少复发率方面,中药内外并治疗法优于曲安奈德益康唑乳膏,中药溻渍法较普通中药熏蒸在降低远期复发率方面更有优势。急性肛周湿疹的患者应把握标本兼治的原则,抓住病因,重视内外联合疗法,发挥中药溻渍等传统疗法的特色与优势,寻求控制急性炎症反应及降低远期复发率更佳的治疗方案。

Objective： To observe the clinical effect of traditional Chinese medicine oral administration combined with Chi- nese herb hydrotherapeutics for treating acute perianal eczema. Methods： 180 patients with eczema acute anal damp confirmed a completely randomly divided into three groups, 60 patients in the hydrotherapeutics group, 60 patients in the fumigation group, 60 treated patients in the controlled group. The treatment lasted 10 days, and to compare the clinical efficacy of the 3 groups of pa- tients and recurrence rate. Results： After treatment, patients in the perianal itching, anal moisture, repair and so relieve the symptoms of skin lesions, while the recurrence rate in the treatment group was less than the controlled group, the difference was statistically significant. Conclusion： Using Qushi Zhiyang External-use Recipe with hydrotherapeutics combined with oral Long- dan Xiegan Decoction in the treatment of acute anal eczema is safe, effective, and it is better than western medicine in improving patient perianal itching, anal damp, skin lesions repair and reduce the recurrence rate advantage. Patients with acute eczema should grasp the specimen and worthy principle, seize the cause, attention inside and outside combination therapy, characteristics and advantages of Chinese medicine and to play a role in Chinese herb hydrotherapeutics and other traditional therapies, seek bet- ter treatment options.
