经腹超声联合经阴道超声对卵巢肿瘤的诊断价值

Diagnostic value of transabdominal ultrasonography combined with transvaginal ultrasonography for ovarian tumors

目的探讨经腹超声联合经阴道超声检查对卵巢肿瘤的诊断价值。方法选取2012年6月至2016年6月间海南省中医院收治的80例卵巢肿瘤患者作为研究对象,分别对患者进行经腹超声、经阴道超声检查以及联合检查,观察比较3种检查方式的诊断结果,并与病理诊断结果进行比较。结果经腹超声联合经阴道超声检查对恶性肿瘤的诊出率(23.8%)高于经腹部超声检查恶性卵巢肿瘤诊出率(15.0%)以及经阴道超声检查恶性卵巢肿瘤诊出率(17.5%),差异均有统计学意义(均P<0.05)。与病理分期结果相比,对于Ⅲ、Ⅳ期恶性肿瘤,联合检查的诊断符合率分别为87.5%和85.7%,明显高于单独经腹超声或经阴道超声检查,差异均有统计学意义(均P<0.05)。良性、恶性卵巢肿瘤血流动力学最大峰值(PSV)、平均流速(Vm)、搏动指数(PI)和阻力指数(RI)等指标进行比较,差异均有统计学意义(均P<0.05)。结论经腹超声联合经阴道超声检查对卵巢肿瘤的诊断价值高,尤其对恶性肿瘤的诊断,值得广泛推广。

Objective To investigate the significance of transabdominal combined with transvaginal ultrasonography in the diagnosis of ovarian tumors. Method A total of 80 patients with ovarian tumors treated at Hainan Traditional Chinese Medicine Hospital from June 2012 to June 2016 were selected as the research object. Transabdominal and transvaginal ultrasonography alone and the combined screening were performed on the patients respectively and the diagnosis results were compared,and were compared with the results of pathological diagnosis. Results The detection rate of malignant tumors was higher in the combined group（ 23. 8%） than in the abdominal ultrasonography group（ 15. 0%） and vaginal ultrasonography group（ 17. 5%）（ all P〈 0. 05）. Compared with pathological staging results,the combined diagnosis rates was87. 5% and 85. 7% for stage III and IV respectively which was significantly higher than that of single diagnosis（ P〈 0. 05）. There was significant difference in blood flow dynamics of benign and malignant ovarian tumors including peak systolic velocity（ PSV）,mean velocity（ Vm）,pulsatility index（ PI） and resistance index（ RI） and other indicators between the two groups（ P〈 0. 05）. Conclusion The combination of transabdominal and transvaginal ultrasonography is of high value in the diagnosis of ovarian tumors,especially for the malignant tumors.
