卡孕栓与米索前列醇宫颈预处理方式在Ⅰ型子宫黏膜下肌瘤电切除术中的效果比较

Efficacy comparison between pretreatment method of carboprost suppository and misoprostol in applications of hysteroscopic transcervical resection of type Ⅰ uterine submucous myoma

目的比较卡孕栓和米索前列醇两种不同宫颈预处理方式在I型子宫黏膜下肌瘤电切除术中的应用效果。方法回顾性分析2013年3月至2016年3月于重庆三博江陵医院接受治疗的146例I型子宫黏膜下肌瘤患者的临床资料,术前采用卡孕栓进行宫颈预处理的为研究组,术前采用米索前列醇进行宫颈预处理的为对照组,每组各73例。观察两组患者围术期相关指标、宫颈松弛情况及药物的不良反应等差异。结果研究组宫颈口平均松弛宽度[(7.62±0.94)mm]及总有效率(94.52%)高于对照组[(6.59±1.21)mm,89.04%],手术时间[(28.59±8.92)min]及术中出血量[(22.49±12.39)m L]少于对照组[(35.34±11.32)min,(38.19±11.59)m L],差异均有统计学意义(P<0.05);两组患者术后均无并发症发生,术后排气时间、用药后药物不良反应差异均无统计学意义(P>0.05)。结论卡孕栓用于宫腔镜电切除术前宫颈预处理效果显著,能有效软化宫颈口,且操作方便,出血量少、用药安全、不良反应小,值得临床推广应用。

Objective To compare the effect of pretreatment method of carboprost suppository and misoprostol in applications of hysteroscopic transcervical resection of type I uterine submucous myoma. Methods Data of 146 patients received hysteroscopic surgery in Chongqing Sanbo Jiangling Hospital from March 2013 to March 2016 were analyzed retrospectively. 73 cases treated with pretreatment of carboprost suppository were selected as study group, while others treated with misoprostol pretreatment were selected as control group. The differences of perioperative indexes, cervical relaxation and adverse drug reactions were observed and compared. Results The average relaxation width of cervix and total effective rate of the study group [ （7. 62 ± 0. 94） mm, 95.42% ] were significantly higher than those of the control group[ （6. 59 ± 1.21 ）mm,89. 04% ] （P 〈0. 05）. The time of operation and blood loss of the study group [ （28.59 ± 8.92） rain, （22. 49± 12. 39） mL ] were significantly less than those of the control group [ （ 35.34 ± 11.32 ） min, （38. 19±11. 5）mL] （P 〈 0. 05）. There were no significant differences between the two groups in complication, postoperative ventilation time and adverse drug reactions （ P 〉 0. 05 ）. Conclusion The effect of carboprost suppository on hysterectomy before cervical resection was significant, which can effectively soften the cervix, and with simple operation, less bleeding, and safety, it is worthy of clinical application.
