摘要
目的探讨2型糖尿病患者体质指数(BMI)与胰岛素抵抗和胰岛β细胞功能的关系。方法根据BMI将376例2型糖尿病患者分为正常体重组、超重组和肥胖组。分别测定其空腹胰岛素(FINS)浓度、空腹血糖(FPG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、甘油三酯(TG)、收缩压(SBP)、舒张压(DBP)、腰围(WC)、臀围、身高和体重,计算胰岛β细胞功能指数(HOMA-β)、胰岛素抵抗指数(HOMA-IR)、腰臀比(WHR)、腰身比(WHtR)及体质指数(BMI),运用SPSS 21.0进行统计学分析。结果 3组研究对象在年龄、饮酒、SBP、DBP、WHR、WHtR、FINS、HOMA-IR及HOMA-β上,差异有统计学意义(P<0.05);超重组和肥胖组WHR、WHtR、FINS、DBP、HOMA-IR、HOMA-β均高于体重正常组(P<0.05),体重正常组的年龄显著大于超重组和肥胖组(P<0.05);肥胖组的SBP、TG、HDL-C也显著高于正常体重组(P<0.05)。Pearson相关分析显示,BMI与FINS、WHR、WHtR、HOMA-IR、HOMA-β、SBP、DBP和TG呈正相关(P<0.05)。结论随着T2DM患者体质指数的增加,胰岛素抵抗水平逐渐升高;超重和肥胖患者胰岛β细胞功能亢进,与正常组比较差异均具有统计学意义。
Objective To explore the correlation of BMI and insulin resistance, islet Henan Province β - cell function among type 2 diabetic patients. Methods According to BMI, 376 type 2 diabetic patients were divided into three groups, including obesity group, overweight group and normal body weight group. Fasting insulin ( FINS), fasting plasma ( FPG), high density lipoprotein cholesterol (HDL - C ), low density lipoprotein cholesterol (LDL - C), total cholesterol ( TC ) and tri- glyceride (TG) were detected; diastolic blood pressure (DBP), systolic blood pressure (SBP), waist circumference (WC), hip circumference, height and body weight were measured and body mass index (BMI), waist -to - hip ratio ( WHR), waist - to - height ratio ( WHtR), HOMA - IR and HOMA - β were computed. We used SPSS 21.0 to compare the data of every group. Results Compare with the age, alcohol drinking, SBP, DBP, WHR, WHtR, FINS, HOMA - IR and HOMA - 15had significant differences among the three groups (P 〈 0. 05). The levels of WHR, WHtR, FINS, DBP, HOMA - IR, HOMA - β in obesity group and overweight group were higher than those in normal weight BMI group ( P 〈 0. 05 ). Compared with the normal body weight group, SBP, TG, HDL - C were also significantly higher (P 〈 0. 05 ). The age of normal weight group were older than obesity group and overweight group ( P 〈 0. 05 ). SBP, TG, HDL - C of obesity group were significant- ly higher than normal weight group ( P 〈 0.05 ). The Pearson correlation analysis demonstrated that BM1 was positively correlated with FINS, WHR, WHtR, HOMA - IR, HOMA - β, SBP, DBP and TG (P 〈 0.05). Conclusion With the increase of BMI of T2 DM patients, insulin resistance has become more severe. The islet - cell function of both overweight and obesity group are hyper and there are significant differences between o- verweight, obesity groups and normal body weight one.
