青年人体质指数、代谢综合征与血压水平的相关性分析 被引量:4

摘要 目的:分析青年人群体质指数(BMI)、代谢综合征(MS)与血压水平的相关性,为高血压制订早期防治提供科学依据。方法:选择2015年某省级医院体检人群中青年人27 043名,询问病史,测定身高、体重、腹围、血压,检验生化指标;按是否患有MS分为MS(+)组和MS(-)两组,按年龄、性别描述其血压和BMI、MS的分布情况;采用Pearson相关性分析检验BMI、MS与血压之间的相关性,采用多分类Logistic回归模型计算BMI、MS对高血压发病风险的预测作用。结果:青年男性BMI、收缩压和舒张压均高于女性,男性和女性均随年龄段增加而呈上升趋势,男性高血压患病风险和MS患病率均高于女性。相关分析结果显示,BMI与收缩压和舒张压均呈正相关(r=0.460,P<0.01;r=0.462,P<0.01)。MS与收缩压、舒张压相关(r=0.388,P<0.01;r=0.400,P<0.01);Logistic回归分析结果显示,体重过低者发生血压正常高值、1级、2级和3级高血压的风险低于正常体重者,超重者发生上述高血压的风险高于正常体重者,肥胖者发生上述高血压的风险为最高;MS(+)人群发生血压正常高值、1级、2级和3级高血压的风险明显高于MS(-)人群。结论:青年人群中超重肥胖和MS(+)均会增加高血压的患病风险,预防和控制高血压时应更注重男性超重肥胖和MS(+)人群,从源头遏制高血压的发生和发展。 Objective: To analyze the correlation between BMI, MS and blood pressure for young people, aiming to provide scientifie support for early prevention and treatment of hypertension.Method: 27 043 young people took physical examinations in a provincial hospital in 2015 were classified into MS ( + ) group and MS ( - ) group according to whether had metablic syndrome, recorded their height, weight, waist circumference, blood pressure, and biochemical indexes.The distributions of blood pressure and BMI, MS were described by age and gender.Pearson's Correlation Analysis was used to analyze the correlations between BMI, MS and blood pressure, a muhinomial logistic model was used to forecast hypertension risk by BMI and MS.Result: BMI, systolic and diastolic blood pressure of young men were higher than those of young women, and increased with age for both genders.The risks of hypertension and MS of man were higher than those of woman.Correlation analysis showed that BMI was positively correlated to systolic and diastolic blood pressure (r=0.460, P〈0.01; r=0.462, P〈0.01 ), and MS as well ( r=0.388, P〈0.01 ; r=0.400, P〈0.01 ) .The multinomial logistic model showed that the risks of pre-hypertension and grade 1, 2, 3 hypertension for underweight were lower than those of normal weight group, the risks for overweight were higher than normal weight, and the risks of MS ( + ) group were significantly higher than that of MS ( - ) group.Conclusion: Overweight and MS ( + ) can result in higher risks of hypertension for young people.Early prevention and treatment of hypertension should focus on male overweight and MS ( + ) to keep hypertension under control from source.
