B超引导下颈内静脉血透长期管置管术的临床运用

Clinical application of internal jugular vein catheterization with long term guided by B ultrasound

目的探讨B超引导下颈内静脉血透长期管置管术的临床应用价值。方法选择72例接受颈内静脉血透长期管置管术的患者作为研究对象,选择治疗时间为2016年3月~2016年9月,采用随机数字抽取表法将患者分为两组,其中对照组患者采实施常规定位后颈内静脉血透长期管置管盲穿,共36例,研究组实施B超引导下颈内静脉血透长期管置管,对两组患者的置管成功率及并发症发生情况进行比较。结果研究组患者首次穿刺成功率为94.44%,总穿刺总功率为100.00%,显著高于对照组首次穿刺成功率55.56%,总成功率75.00%,差异具有统计学意义(P<0.05)。研究组并发症发生率为0.00%,对照组并发症发生率为19.44%,差异具有统计学意义(P<0.05)。研究组患者穿刺时间显著低于对照组,差异具有统计学意义(P<0.05)。结论对颈内静脉血透长期管置管术的患者采用B超引导下置管成功率更高,且并发症发生率明显降低,具有明显的安全性,值得在临床上推广应用。

Objective To investigate the ultrasound guided internal jugular vein intubation in long-term hemodialysis clinical value. Methods 72 patients receiving long-term hemodialysis as internal jugular vein intubation in patients with the object of study, according to the random number table extraction method the patients were divided into two groups, the patients in the control group implemented conventional localization after the internal jugular vein in long-term hemodialysis tube blind, a total of 36 cases, the study group received ultrasound guided internal jugular vein in longterm hemodialysis tube, the two groups of patients with the success rate of catheterization and complications were compared. Results Group The success rate of first puncture patients was 94.44%, the total power of the total puncture was 100.00%, significantly higher than the control group for the first time puncture success rate of 55.56%, the total success rate of 75.00%, the data was statistically significant(P<0.05). The incidence of complications was 0.00%, the complication rate of control group was 19.44%, the data was statistically significant(P<0.05). The puncture time of patients in the study group was significantly lower than the control group, the data was statistically significant(P<0.05). Conclusion The internal jugular vein intubation in long-term hemodialysis patients by ultrasound guided the success rate of intubation was higher, and the incidence of complications was significantly lower, with obvious safety, it is worthy of clinical application.
