贯穿缝合加压包扎在耳廓假性囊肿中的应用

Application of Transfixion Compression Bandaging in Pseudocyst of Auricle

目的分析耳廓假性囊肿前壁软骨切除后贯穿缝合加压包扎治疗耳廓假性囊肿的效果。方法回顾分析我科于2014年1月—2016年3月112例耳廓假性囊肿手术患者,其中45例采用贯穿缝合加压包扎法,67例采用传统手术方式,观察患者积液形成量、术后换药次数、皮肤愈合、耳廓外形、有无复发等情况,比较两组在换药次数及愈合时间的差别。结果贯穿缝合加压组仅5例出现耳廓少许积液,无皮肤坏死,大多数中间只需来院拆除加压包扎1次,伤口一期甲级愈合,6～7天可拆线,未见畸形,无复发。传统手术组1例出现了皮肤坏死予二期修复,其余66人均需换药3次左右,伤口一期甲级愈合,7天左右拆线,未见畸形,无复发,两组换药次数及愈合天数对比,差异有统计学意义(P<0.01)。结论采用切除前壁软骨后贯穿缝合加压包扎术治疗耳廓假性囊肿效果良好,术区不易形成积液,术后换药次数少,愈合时间短,患者痛苦减少,耳廓局部皮肤无增厚、无畸形。

Objective To analyze the effect of suture compression bandaging in the treatment of pseudocyst of auricle after anterior wall resection of pseudocyst of auricle. Methods From January 2014 to March 2016, 112 cases of pseudocyst of auricle in our department was performed, among them, 45 cases were treated by transfixion suture compression bandaging method, and 67 cases by traditional operation, the volume of fluid, the number of dressing change, the healing of skin, the shape of auricle and the recurrence of the disease were observed, the difference of dressing frequency and healing time between the two groups was compared. Results In the suture compression group, only 5 cases had a little effusion and no skin necrosis. Most of the patients only needed to remove the pressure bandage 1 time in the middle, the wound healed at first grade, and the stitches could be removed in 6 to 7 days, no deformity and no recurrence occurred. In the operation group, 1 case had skin necrosis and two stage repair. The remaining 66 patients needed dressing change about 3 times. The wound healed at first stage, and the stitches were removed in 7 days. No deformity or recurrence was found,the number of dressing change and healing days of the two groups were compared, the difference was statistically significant（P〈 0.01）. Conclusion After the excision of anterior wall cartilage, the suture compression bandaging method has good effect on the treatment of pseudocyst of auricle, and the operation area is not easy to form effusion. After the operation, the number of dressing change is short, healing time is short, the pain of patients is reduced, and the local skin of auricle has no thickening and no deformity.
