IL-6在PROM孕妇中的表达及与破膜时间和宫内感染的关系

Expression of serum IL-6 in pregnant women with premature rupture of membranes and its relationship with time of rupture of membrane and intrauterine infections

目的观察血清白细胞介素6(IL-6)在胎膜早破(PROM)孕妇中的表达,探讨其与破膜时间和宫内感染的关系。方法选取2014年1月-2016年12月于医院妇产科检查并住院分娩的200例孕妇为研究对象,其中PROM孕妇140例为实验组,足月未胎膜早破的正常分娩孕妇60例为对照组;实验组包括足月前胎膜早破(PPROM)孕妇70例和足月胎膜早破(TPROM)孕妇70例;比较两组患者临床指标。结果实验组血清WBC、CRP和IL-6表达均高于对照组(P<0.05);实验组宫内感染率29.3%,高于对照组宫内感染率1.7%(P<0.05);实验组中合并宫内感染者血清WBC、CRP和IL-6表达均高于无宫内感染者(P<0.05);TPROM合并宫内感染者血清WBC和CRP表达高于无宫内感染者(P<0.05);PPROM合并宫内感染者血清WBC、CRP和IL-6表达高于无宫内感染者(P<0.05);与破膜时间<24h的孕妇比较,宫内感染发生率、血清CRP和IL-6表达在破膜时间>24h的孕妇明显增高(P<0.05)。结论血清IL-6水平与PROM孕妇宫内感染、破膜时间长短密切相关。

OBJECTIVE To observe the expression of serum interleukin-6（IL-6）in patients with premature rupture of membranes（PROM）and its relationship with time of rupture of membrane and intrauterine infection.METHODS A total of 200 pregnant women who were treated with hospital delivery and regular prenatal detection from Jan.2014 to Dec.2016 in our department of gynaecology and obstetrics were enrolled in this study,including140 cases with PROM referred as study group,and 60 normal delivery cases without PROM referred as control group.The study group included preterm premature rupture of membrane（PPROM）pregnant women（70 cases）and term premature rupture of membrane（TPROM）pregnant women（70 cases）.Clinical indexes were compared between the two groups.RESULTS The levels of serum WBC,CRP and IL-6 in study group were significantly higher than those in control group（P〈0.05）.The intrauterine infection rate of study group was 29.3%,which was significantly higher than 1.7% of control group（P〈0.05）.The levels of serum WBC,CRP and IL-6 of the cases in study group complicated with intrauterine infections were significantly higher than those of the cases without intrauterine infection（P〈0.05）.The levels of WBC and CRP in the cases of TPROM group complicated with intrauterine infections were significantly higher than those in the cases without intrauterine infection（P〈0.05）.The levels of serum WBC,CRP and IL-6 in the cases of PPROM group complicated with intrauterine infections were significantly higher than those in the cases with intrauterine infection（P〈0.05）.Furthermore,compared with the pregnant women with time of rupture of membrane below 24 hours,the intrauterine infection rate and the levels of serum CRP and IL-6 in pregnant women with time of rupture of membrane above 24 hours were significantly increased（P〈0.05）.CONCLUSIONThe level of serum IL-6 is closely correlated with intrauterine infections and the time of rupture of membrane.
