针刺联合高压氧治疗运动区颅脑外伤术后偏瘫48例疗效观察

Clinical observation of acupuncture combined with hyperbaric oxygen on 48 cases with postoperative limbs hemiplegia of craniocerebral trauma in motor area

目的观察针刺联合高压氧治疗颅脑外伤术后偏瘫的临床疗效。方法将96例颅脑外伤术后偏瘫患者随机分为2组,对照组48例予常规抗炎、降颅内压、营养等治疗,治疗组48例在对照组治疗基础上加用针刺联合高压氧治疗。2组均10 d为1个疗程,疗程间休息3 d,3个疗程后统计临床疗效,并观察2组治疗前后上、下肢神经功能缺损程度评分,简化Fugl-Meyer运动功能评分,简化Fugl-Meyer平衡功能评分,Brunnstrum分级评定及Barthel相关指数评价量表评分(BI评分)情况。结果治疗组总有效率47.92%,对照组总有效率22.92%,2组比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。治疗组治疗后上、下肢神经功能缺损程度评分,简化Fugl-Meyer运动功能评分,简化Fugl-Meyer平衡功能评分均改善,与本组治疗前比较差异均有统计学意义(P<0.05)。对照组治疗后简化Fugl-Meyer运动功能评分改善,与本组治疗前比较差异有统计学意义(P<0.05)。2组治疗后上、下肢神经功能缺损程度评分,简化Fugl-Meyer运动功能评分,简化Fugl-Meyer平衡功能评分比较差异均有统计学意义(P<0.05)。治疗组治疗后Brunnstrum分级Ⅰ级、Ⅱ级例数明显减少,Ⅴ级、Ⅵ级例数明显增多,比较差异有统计学意义(P<0.05)。对照组治疗前后Brunnstrum分级例数分布比较差异无统计学意义(P>0.05)。2组治疗后BI评分均改善(P<0.05),且治疗组治疗后BI评分改善优于对照组(P<0.05)。结论针刺联合高压氧对颅脑外伤术后偏瘫患者的康复疗效肯定。

Objective To observe the effects of acupuncture combined with hyperbaric oxygen on patients with postoperative limbs hemiplegia of craniocerebral trauma. Methods 96 patients with postoperative limbs hemiplegia of craniocerebral trauma were divided into two groups. 48 cases in the control group were treated by routine treatment including anti-inflammatory,reduced intracranial pressure and nutrition. 48 cases in the treatment group were treated by acupuncture combined with hyperbaric oxygen on the basis of the control group. A treatment course was 10 d,3 d rest between the two courses. The curative effects were evaluated after three courses. The upper and lower limbs neurological function deficit scale,simplify Fugl-Meyer motor function score,simplified Fugl-Meyer balance function score,Brunnstrum grade and Barthel( BI) score before and after treatment were observed in two groups. Results The total effective rate in the treatment group( 47. 92%) was significantly higher than that in the control group( 22. 92%,P < 0. 05),and the effect in the treatment group was better than that in the control group.The scores of the upper and lower limbs neurological function deficit scale,simplify Fugl-Meyer motor function score and simplified Fugl-Meyer balance function score after treatment were improved in the treatment group( P < 0. 05).There were statistical differences on the simplify Fugl-Meyer motor function score between before and after treatment in the control group( P < 0. 05). After treatment,there were statistical differences on the upper and lower limbs neurological function deficit scale,simplify Fugl-Meyer motor function score and simplified Fugl-Meyer balance function score between the two groups( P < 0. 05). After treatment,the number of the Brunnstrum grade Ⅰ and grade Ⅱ significantly decreased in treatment group,while the number of grade Ⅴ and grade Ⅵ significantly increased( P <0. 05). There was no statistical difference on the number of Brunnstrum grade between before and after treatment in the control group( P > 0. 05). The BI scores after treatment were improved in two groups,and which in the treatment group was superior to that in the control group( P < 0. 05). Conclusion Acupuncture combined with hyperbaric oxygen has exact effects patients with postoperative limbs hemiplegia of craniocerebral trauma.
