自拟滋阴补气活血方对气阴两虚兼血瘀型IgA肾病患者D-二聚体、T细胞亚群及临床疗效影响

Effects of Selfmade Ziyin Buqi Huoxue Decoction on D-two Dimer,T Lymphocyte Subsets and Clinical Efficacy in Patients with Ig A Nephropathy with Qi and Yin Deficiency and Blood Stasis Syndrome

目的 ：探讨自拟滋阴补气活血方对气阴两虚兼血瘀型IgA肾病患者D-二聚体、T细胞亚群及临床疗效的影响。方法 ：将本院收治的122例气阴两虚型IgA肾病（CKD 1-3期）患者随机分为对照组（n=60）和观察组（n=62）,对照组予以常规西医治疗,观察组在西医常规治疗基础上予以自拟滋阴补气活血方治疗,比较2组患者的D-二聚体、T细胞亚群及临床疗效。结果 ：治疗后,观察组及对照组的内生肌酐清除率均明显高于治疗前,且观察组明显高于对照组;治疗后2组患者的24 h尿蛋白定量及尿沉渣红细胞计数均明显低于治疗前,且观察组明显低于对照组,均P〈0.05。观察组及对照组的D-二聚体均明显低于治疗前,且观察组明显低于对照组,P〈0.05。治疗后,2组患者的CD4^＋/CD8^＋、NK细胞比较差异无统计学意义,P〉0.05;对照组及观察组的CD3^＋、CD4^＋水平均明显高于治疗前,且观察组明显高于对照组,均P〈0.05。对照组的总有效率为63.33%,观察组的总有效率为85.48%,经χ^2检验,观察组的总有效率明显高于对照组,P〈0.05。结论 ：在气阴两虚型IgA肾病患者中采用自拟滋阴补气活血方治疗,能有效改善患者的细胞免疫及体液免疫,降低蛋白尿、血尿,提高临床疗效,具有重要的临床意义。

Objective ：To investigate the effects of Ziyin Buqi Huoxue Decoction on D-two dimer,T lymphocyte subsets and clinical efficacy in IgA nephropathy patients with Qi and Yin deficiency and blood stasis syndrome. Methods ：122 cases of IgA nephropathy（stage CKD 1-3）were randomly divided into the control group（n=60）and the observation group（n=62）,the control group received routine Western medicine treatment,the observation group was treated with Ziyin Buqi Huoxue Decoction,the clinical efficacy was compared between the two groups. Results ：After treatment,the creatinine clearance rate in the observation group and the control group were significantly higher than that before treatment. After treatment,the two groups of patients with 24 h urine protein and urine sediment red blood cell count were significantly lower than before treatment,in the observation group and control group,D-two dimer were significantly lower than before treatment;After treatment,there was no significant difference in CD4^＋/CD8^＋and NK cells between the 2 groups,P〈0.05;The total effective rate of the control group was 63.33%,and the total effective rate of the observation group was 85.48%,the total effective rate of the observation group was significantly higher than that of the control group,P〈0.05. Conclusion ：It can effectively improve the cellular immunity and humoral immunity of patients,reduce proteinuria, hematuria,and improve the clinical efficacy,which has important clinical significance. is effective on treating IgA nephropathy patients of Qi and Yin deficiency type.
