摘要
目的探讨荧光引导技术在高级别胶质瘤手术治疗中的优势与价值。方法回顾性分析2015年1月—2016年6月收治的30例高级别胶质瘤患者的临床资料,其中16例在荧光素钠显影引导下行手术治疗,术中在显微镜荧光模式下,根据荧光显影的范围进行肿瘤边界判定、切除,对不同肿瘤区域的的荧光强度和分子病理标志物表达进行比较;14例行传统手术治疗,常规送病理检查;两组病例术后3天内常规行MRI增强扫描,根据强化范围估算切除程度并进行对比。结果荧光素钠组肿瘤全切率为87.5%,传统手术组肿瘤全切率为50.0%,差异具有统计学意义(P<0.05)。结论荧光素钠显影技术可以帮助高级别胶质瘤术中更加准确的判定肿瘤边界,提高全切率,明显延长患者的术后生存时间,提高患者的生存质量。
Objective To explore the advantages and value of fuorescenceguided technology in high-grade glioma operative treatment. Methods Thclinical data of 30 patients with high-grade glioma from January 2015 toJune 2016 were retrospectively analyzed. Among them, 16 cases performedoperative treatment under the guidance of fluorescein sodium imagingDuring the operation treatment and under microscope fuorescence modefluorescence imaging range was performed to determine tumor bordeand excision. Compared different tumor regions fluorescence intensitywith molecular pathological marker expression. 14 cases were treated byconventional surgery and routinely sent for pathological examination. Twogroups of operative patients three days later routine MRI Enhance the scanaccording to strengthen the scope, estimated the degree of resection andcompared. Results In the sodium fuorescein group, the total resection ratwas 87.5%, while the total resection rate in the conventional surgery groupwas 50.0%. The difference has statistical signifcance (P〈0.05). ConclusionSodium fuorescein imaging technology can help high-grade glioma surgerymore accurately determine the tumor boundary, improve the rate of fullcut, signifcantly prolong the survival time of patients and improve the lifquality of patients.
