摘要
目的探讨同伴教育对永久性结肠造口患者生活质量及社会心理适应状况的影响。方法 2015年1月至2016年6月,便利抽样法选择在某院治疗的结肠造口患者113例为研究对象,采用抽签法将其分为观察组(n=56)和对照组(n=57)。对照组患者采用常规的健康教育形式,观察组患者进行同伴教育。干预前后采用欧洲癌症研究与治疗组织的生活质量核心量表(European Organization for research and treatment of cancer quality of life queationnaire core-30,EORTC QLQ-C30)和结肠造口患者社会心理适应量表对患者调查并比较。结果干预前,所有患者QLQ-C30功能维度中,角色功能、情绪功能和社会功能得分低于常模,差异均有统计学意义(均P<0.05);与症状相关的维度方面,患者疲劳、食欲下降、失眠、腹泻和经济困难感知得分高于常模,差异均有统计学意义(P<0.05)。两组患者QLQ-C30量表和结肠造口患者社会心理适应量表各维度的得分差异均无统计学意义(均P>0.05);干预后,在QLQ-C30功能维度中,观察组患者在角色功能、情绪功能、认知功能和社会功能得分升高;与症状相关的维度方面,观察组患者失眠、便秘和腹泻的得分降低;在整体健康状况方面,观察组患者的得分升高,与对照组比较,差异均有统计学意义(均P<0.05)。观察组在结肠造口患者社会心理适应量表的正性情绪、负性情绪和社会生活适应维度方面的得分升高,与对照组的差异均有统计学意义(均P<0.05)。结论同伴教育在改善永久性结肠造口患者的生活质量和社会心理适应状况的过程中起到了积极的作用,值得进一步推广使用。
Objective To analyze the effects of peer-led education on quality of life and psychosocial adaptation of patients with permanent colostomy.Methods By convenience sampling,113 patients with permanent colostomy were selected and randomly divided into observation group(n=56)and control group(n=57).The control group received routine health education mode,while the observation group received peer-led education.Before and after the intervention,the European Organization for research and treatment of cancer quality of life queationnaire core-30(EORTC QLQ-C30)and ostomy adjustment inventory scores were collected,and data were analyzed.Results Before the peer-led education,the scores of role function,emotional function and social function of QLQ-C30 were significantly lower than the norm(all P<0.05);while the scores of fatigue,decreased appetite,insomnia,diarrhea and economic difficulty of QLQ-C30 were significantly higher than the norm(P<0.05).Before the peer-led education,all of the factor scores of QLQ-C30 and ostomy adjustment inventory were not significantly different between two groups(all P>0.05),while after the peer-led education,the scores of role function,emotional function,cognitive function and social function were statisticallyimproved.The insomnia,constipation and diarrhea in observation roup were decreased.The score of general health in observation group was increased(all P<0.05).The scores of positive emotion,negative emotion and social adaptation of ostomy adjustment inventory were significantly higher than the control group(all P<0.05).Conclusion The peer-led education has positive effect on the quality of life and social mentality adaptation of patients underwent permanent colostomy,which is worth of clinical promotion.
