中药杀胚方联合米非司酮、氨甲喋呤治疗异位妊娠的临床研究

Clinical research on treating ectopic pregnancy with the Shapei decoction plus mifepristone and methotrexate

目的:探讨中药杀胚方联合米非司酮、氨甲喋呤治疗异位妊娠的临床疗效。方法:以我院妇产科收治的240例输卵管移位妊娠患者为研究对象,采用随机数字表分为西医组与中西结合组,每组各120例,其中西医组予以米司非酮、氨甲喋呤联合治疗,中西结合组予以中药杀胚方联合米司非酮、氨甲喋呤治疗。比较两组的临床疗效,并记录两组β-HCG恢复正常、异位妊娠包块吸收及月经复潮所用时间。结果:西医组总有效率为81.67%,明显低于中西结合组的91.67%,差异显著(P<0.05);中西结合组β-HCG恢复时间为(13.6±5.8)d,异位妊娠包块吸收时间为(83.4±14.6)d,月经复潮时间为(75.4±12.3)d,均明显低于西医组,差异均显著(P<0.05);西医组不良反应总发生率为10.00%,与中西结合组的6.67%比较无显著差异(P>0.05)。结论:中药杀胚方联合米非司酮、氨甲喋呤治疗输卵管异位妊娠临床疗效显著,可有效促进患者β-HCG恢复正常及附件包块吸收,且未增加不良反应,安全性较高,值得临床推广。

Objective： To explore clinical efects of the Shapei decoction plus mifepristone and methotrexate on ectopic pregnancy. Methods： 240 cases in our hospital were divided into the western medicine group and the combination group, 120 cases in each. The western medicine group took mifepristone plus methotrexate. The combination group received the Shapei decoction more. Results： The total efciency in the western medicine group was 81.67%, signifcantly lower than 91.67% in the combination group （P〈0.05）. In the combination group, the recovery time of β-HCG was （13.6±5.8） days, ectopic pregnancy mass absorption time was （83.4±14.6） days, menstrual complex tide time was （75.4±12.3） days, significantly lower than those in the western medicine group （P〈0.05）. The total incidence of adverse reactions in the western medicine group was 10.00%, there was no signifcant diference compared with 6.67% in the combination group （P〉0.05）. Conclusion： The therapeutic efects of the Shapei decoction plus mifepristone and methotrexate on ovarian ectopic pregnancy is remarkable, could efectively promote the recovery of β-HCG and the absorption of the attached mass, did not increase the adverse reaction, with safety, and is worthy of clinical promotion.
