TCD在脑梗死患者脑血流参数变化中的诊断价值研究

Study on the diagnostic significance of changes in cerebral blood flow parameters in patients with cerebral infarction

目的研究与分析经颅多普勒(TCD)在脑梗死患者脑血流参数变化中的诊断价值。方法前瞻性选取2015年6月至2017年2月收治的65例脑梗死患者为观察组,65例体检健康者为对照组。检测与比较两组的大脑前动脉、中动脉及后动脉脑血流参数,比较观察组中不同分期及严重程度者的脑血流参数。结果观察组的大脑前动脉、中动脉及后动脉平均血流速度(Vm)低于对照组,阻抗指数(RI)及搏动指数(PI)高于对照组,观察组患者急性期Vm低于亚急性期,RI及PI高于亚急性期,且NIHSS评分较高者的Vm低于评分较低者,RI及PI高于评分较低者,差异均有统计学意义(P<0.05)。结论 TCD在脑梗死患者脑血流参数变化中的诊断价值较高,对不同分期及严重程度者的脑血流参数也有区别意义,因此对疾病的诊治具有指导作用。

Objective To study and analyze the diagnostic significance of TCD in changes of parameters in cerebral blood flow of patients with cerebral infarction. Methods A total of 65 patients with cerebral infarction in this hospital during June 2015 to February 2017 were prospectively selected as observation group,and 65 healthy persons for physical examination were selected as control group. Then the parameters of blood flow in anterior,middle and posterial cerebral arteries in these 2 groups were detected and compared,and the parameters of cerebral blood flow in cerebral arteries of patients in observation group with different stages and degree of severity were compared. Results The mean blood flow velocities（ Vm） in anterior cerebral artery,middle cerebral artery and posterior cerebral artery in observation group were all lower than those of control group,the resistent index（ RI） and pulsatility index（ PI） were higher than those of control group. The Vm in observation group at acute stage was lower than that at subacute stage,RI and PI were higher than those at subacute stage,the Vm in patients with higher NIHSS scores were lower than those of patients with lower scores,the RI and PI were higher than those of patients with lower scores,and the difference was statistically significant（ P〈0. 05）. Conclusion The diagnostic significance of TCD in changes of parameters of cerebral blood flow in patients with cerebral infarction is higher,and it has distinctive significance for cerebral blood flow parameters at different stages and degree of severity,so it has important significance for guidance of the diagnosis and treatment of patients with this kind of disease.
