摘要
目的探讨结肠癌病灶最大标准化摄取值(SUVmax)与KRAS基因突变及患者临床特征的关系。方法回顾性分析45例术后病理证实为结肠癌患者的PET/CT资料,分别按患者年龄、性别、病灶部位、TNM分期、病理类型及组织分化程度、癌胚抗原(CEA)、糖类抗原199(CA199)、糖类抗原125(CA125)、KRAS基因突变状态进行分组,分析各组间SUVmax值差异有无统计学意义。结果 KRAS突变率33.3%(15/45),KRAS突变组SUVmax(16.9±14.1)高于野生组(13.0±9.8),差异具有统计学意义(P〈0.05)。KRAS突变与性别、年龄、病变部位、肿瘤长径、病理类型、组织分化程度、AJCC分期无明显相关(P〉0.05)。SUVmax与患者性别和年龄、病灶部位、病理类型、CA199、CA125无明显相关(P〉0.05),与病灶的长径、TNM分期、AJCC分期、组织学分化程度、CEA有关(P〈0.05)。结论结肠癌SUVmax偏高可能与KRAS突变有一定关系,SUVmax也可能与病灶增殖及侵袭能力有关。
Objective To explore the relationship between the maximum standardized uptake value(SUVmax) and KRAS mutation in the patients with colon cancer.Methods PET/CT preoperative images of forty-five patients diagnosed with colon cancer were retrospectively analyzed to evaluate the relationship of SUVmax between KRAS mutation and clinical pathological features.The groups were matched by sex,age,lesion position,diameter,TNM stage,American Joint Committee on Cancer(AJCC) stage,pathological type,differentiation state,carcinoembryonicantigen(CEA),carbohydrate antigen 199(CA199) and carbohydrate antigen 125(CA125),KRAS mutation state and SUVmax was calculated.Results SUVmax of colon cancer with mutational-type KRAS(16.9 ± 14.1) were higher than those with wild-type KRAS(13.0 ± 9.8).There was no significant difference between KRAS mutation and sex,age,lesion position,diameter,AJCC stage,pathological type,differentiation state.There was no correlation between SUVmax and sex,age,lesion position,pathological type CA199 and CA125(P〉0.05).But SUVmax was correlated with lesion diameter,TNM stage,AJCC stage,differentiation state and CEA(P〈0.05).Conclusion High18 F-FDG accumulation can be correlated with KRAS mutation in colon cancer patients.SUVmax can also reflect the invasion and proliferation ability of colon cancer.
