血清胆碱酯酶活性对脑梗死后血管性痴呆的早期预测作用 被引量:10

摘要 目的探讨血清胆碱酯酶活性对脑梗死后血管性痴呆的早期预测作用。方法前瞻性纳入2015年4月到2016年4月在我院住院治疗的急性脑梗死患者及同期非脑梗死患者,搜集整理所有研究对象的一般临床资料及血清胆碱酯酶活性。急性脑梗死发病3个月后对所有患者进行随访,用简易精神状态量表评估认知减退程度,并根据评分将随访患者分为血管性痴呆组和非痴呆组。分析血清胆碱酯酶活性对血管性痴呆的早期预测作用。结果急性脑梗死患者血清胆碱酯酶活性显著下降,其活性与疾病严重程度呈明显负相关。血清胆碱酯酶活性下降与继发血管性痴呆相关,当其值低于6458IU/L时对血管性痴呆的预测敏感度和特异度分别为93.8%和85.3%。结论脑梗死患者急性期血清胆碱酯酶活性可用于预测继发性血管性痴呆的发生。 Objective To determine the function of serum cholinesterase(ChE)activity for predicting post-stroke dementia(PSD)in stroke patients.Methods Consecutive patients and controls suffering other diseases were included in this study from April 2015 to April 2016.Clinical characteristics were collected and fast serum was obtained in the morning after the admission day.Serum ChE activity was determined and the patients had been followed for 3 months after the stroke.Receiver operating characteristic(ROC)curves were obtained and the optimal cut-off points were defined to explore ChE activity in predicting PSD.Results Serum ChE activity had decreased in stroke patients compared with controls.It was inversely correlated with disease severity.Based on ROC curves,the optimal cut-off point was6458 IU/L for predicting PSD,and the sensitivity and specificity values were 93.8% and 85.3%.Conclusion Serum ChE activity can be regarded as an early biomarker for predicting PSD in stroke patients.
