膝骨关节炎急性期关节镜治疗配合穴位注射的临床观察

Clinical Observation of Arthroscopic Treatment Combined with Acupoint Injection in Acute Stage of Knee Osteoarthritis

目的观察膝骨关节炎急性期患者关节镜下膝关节清理术配合穴位注射治疗疗效。方法患者84例随机分为两组,对照组给予关节镜下膝关节清理术,观察组在对照组基础上联合去穴位注射治疗。观察两组疼痛程度、膝关节功能、临床效果以及并发症发生率情况。结果术后两组各时刻VAS评分显著低于术前(P<0.05),且观察组显著低于对照组(P<0.05),两组术后每两个时刻比较差异具有统计学意义(P<0.05);两组术后各时刻Lysholm评分显著高于术前(P<0.05),两组术后1个月、术后3个月差异无统计学意义(P>0.05),术后6个月、术后12个月比较差异有统计学意义(P<0.05);观察组术后各时刻比较差异有统计学意义(P<0.05);对照组术后各时刻差异无统计学意义(P>0.05);观察组优良率为90.48%,显著高于对照组的71.43%(P<0.05);观察组并发症发生率为2.38%,低于对照组的7.14%,但差异无统计学意义(P>0.05)。结论关节镜下膝关节清理术配合穴位注射治疗膝骨关节炎急性期能够有效减轻疼痛,改善膝关节功能,疗效较好。

Objective： To observe the effect of arthroscopic knee debridement combined with acupoint injection on acute knee osteoarthritis. Methods： 84 cases were selected as the research object,and divided into the observation group and the control group,according to the principle of random and equal division. The control group was given arthroscopic knee joint debridement,and the observation group was combined with acupoint injection on the basis. The pain degree,knee joint function,clinical effect and incidence of complications were observed in the 2 groups. Results： The VAS scores of the 2 groups at all times were significantly lower than those before the operation（P〈0.05）,and the observation group was significantly lower than the control group（P〈0.05）;the difference between the 2 groups was statistically significant at every moment after operation（P〈0.05）. The Lysholm scores of the two groups at each time point were significantly higher than those before operation（P〈0.05）;the difference was not significant between the two groups in postoperative 1 st and 3 rd month（P〉0.05）;however,there was significant difference in postoperative 6 th and 12 th month（P〈0.05）;there was significant difference in the observation group after surgery（P〈0.05）,while the difference was not significant in the control group（P〈0.05）. The optimal rate of the observation group was 90.48%,which was significantly higher than that of the control group（71.43%）（P〈0.05）. The incidence of complications of the observation group was 2.38%,lower than that of the control group（7.14%）（P〈0.05）. Conclusion： Arthroscopic knee debridement combined with acupoint injection is effective in relieving pain and improving the function of knee joint in acute stage of knee osteoarthritis.
