沙美特罗替卡松治疗支气管扩张合并不可逆气流受限患者的疗效与安全性分析

Efficacy and safety of salmeterol and fluticasone in the treatment of patients with bronchiectasis and irreversible airflow limitation

目的探讨沙美特罗替卡松对支气管扩张合并不可逆气流受限患者的临床疗效及安全性。方法 86例支气管扩张合并不可逆气流受限患者,采用随机双盲法分为对照组和观察组各43例,对照组采取常规治疗,观察组在对照组基础上采用沙美特罗替卡松进行治疗,对比两组治疗前后生活质量、肺功能改变等情况,同时比较两组随访6个月期间急性发作情况及不良反应发生情况。结果治疗后两组生活质量评分及呼吸困难量表评分均下降,且观察组改善效果优于对照组(P<0.05);治疗前后,两组肺功能各指标比较差异均无统计学意义(P>0.05);观察组急性发作次数低于对照组(P<0.05);两组不良反应发生率差异无统计学意义(P>0.05)。结论支气管扩张合并不可逆气流受限患者采用沙美特罗替卡松治疗可以有效缓解临床症状,提高生活质量,效果显著,值得临床推广。

Objective To investigate the clinical efficacy and safety of salmeterol and fluticasone in the treatment of patients with limited bronchiectasis and limited airflow.Methods Eighty-six patients with bronchiectasis and irreversible airflow limitation were randomly divided into control and observation groups,43 in each group.The control group was given routine treatment while the observation group was treated with salmeterol and fluticasone on the basis of the control group. The clinical symptom score,quality of life and changes of lung function before and after treatment were compared between the two groups.The patients were followed up for 6 months to observe the acute episodes and adverse reactions.Results After treatment,quality of life and dyspnea scale were decreased significantly in the both groups,and the improvement effect in the observation group was significantly better than that in the control group（ P〈0. 05）.Before and after treatment,there was no significant difference in the indexes of lung function between the two groups（ P〈0. 05）.The number of acute attacks in the observation group was significantly lower than that in the control group（ P〈0. 05）. There was no significant difference in the incidence of adverse reactions between the two groups（ P〈0. 05）.Conclusion Salmeterol and fluticasone for the treatment of patients with bronchiectasis and irreversible airflow limitation can effectively alleviate the clinical symptoms and improve the quality of life.It is worthy of clinical promotion.
