中西医结合治疗腰椎间盘突出症的临床研究

Therapeutic effects of integrated traditional chinese and western medicine on lumbar disc herniation

目的探讨以三维正脊牵引为主中西医结合治疗腰椎间盘突出症的临床疗效。方法选择符合纳入诊断标准的腰椎间盘突出症患者150例,随机划分为治疗组和对照组,每组75例。治疗组采用三维正脊牵引治疗,口服自拟强腰益肾汤,配合针刺+骶管冲击的治疗方法。对照组采用口服自拟强腰益肾汤,配合针刺+骶管冲击的治疗方法。三维正脊牵引和骶管治疗每周1次,共3次,口服中药每日1剂,7 d为1个疗程,针刺治疗每2日1次,连续治疗1个月。2组患者均在治疗1个月后对临床疗效、疗效标准评分、疼痛VAS评分比较,进行疗效判定。结果治疗组患者有效率为90.7%,对照组为74.7%,治疗组高于对照组,差异有统计学意义(P<0.05)。治疗组疗效标准评分明显高于对照组评分,差异均有统计学意义(P<0.05),2组患者治疗前后,根据VAS标准评分进行自身比较,差异有统计学意义(P<0.05)。结论三维正脊牵引在腰椎间盘突出症治疗中可显著提高有效率,增强疗效,是综合治疗中的重要方法。采用三维正脊牵引为主的中西医结合治疗腰椎间盘突出症,在短期内明显缓解患者疼痛,改善症状,疗效确切,值得临床推广。

Objective To investigate the therapeutic effects of integrated traditional chinese and western medicine taking three dimensional traction as main method on lumbar disc herniation. Methods A total of 150 patients with lumbar disc herniation were randomly divided into treatment group（n = 75） and control group（n = 75）. The patients in control group were treated by oral Qiangyaoyishen decoction combined with acupuncture and sacral canal injection. On this base,the patients in treatment group were treated by three dimensional traction. The decoction was taken once a day for seven days. The acupuncture treatment was given every two days for a month. Three dimensional traction and sacral canal injection were performed once a week for three times. After treatment of a month,the clinical efficacy,efficacy standard score and visual analogue scale（VAS） score were compared between the two groups. Results The effective rate（90. 7%） in treatment group was significantly higher than that（74. 7%） in control group（P〈0. 05）. The efficacy standard scores in treatment group were significantly higher than those in control group（P〈0. 05）. Moreover,there were significant difference in the visual analogue scale（VAS） scores in both groups before and after treatment（P〈0. 05）. Conclusion The three dimensional traction in treatment of lumbar disc herniation can significantly improve the effective rate,relieve pain in a short time and improve the symptoms,which is worth using widely in clinical practice.
