小儿阴茎感知量表在尿道下裂术后阴茎外观评估中的应用

Application of pediatric penile perception score in assessment of penile appearance after hypospadias surgery

目的初步分析小儿阴茎感知量表(pediatric penile perception score,PPPS)在评估尿道下裂术后阴茎外观中的应用价值。方法以2014年1月至2016年12月由同一泌尿外科医生行尿道板中央切开卷管尿道成形术(tubularized incised plate,TIP)的尿道下裂术后患儿为(实验组),随访中由患儿父母、非手术医生分别独立完成小儿阴茎感知评分量表(PPPS),并对不同受试者给出的PPPS得分进行对比分析。为验证该量表评估尿道下裂术后阴茎外观的结果是否客观,同时选取接受包皮环切术后患儿的照片作为本研究对照组,患儿父母在不知晓照片身份信息的情况下同样采用PPPS对照片中的阴茎外观进行评估。结果共纳入74例患儿,接受手术时年龄6～144个月,平均36个月,术后随访时间1～117个月,平均12个月。PPPS结果显示,泌尿外科医生、患儿父母对尿道下裂术后阴茎外观的满意评分分别为(2. 83±0. 28)分、(2. 32±0. 52)分,非手术医生对尿道下裂术后阴茎外观评分显著高于患儿父母(P <0. 001)。结论 PPPS能客观地用于尿道下裂术后阴茎外观的评估,但医生与患儿父母间评估结果存在一定的差异。

Objective To evaluate the role of pediatric penile perception score （PPPS） in assessment ofpenile appearance af ter hypospadias operations. Methods From January 2014 to December 2016,a total of 74boys underwent tubularized incised plate （TIP） urethroplasty by the same urologist. Their clinical data werecollected and analyzed. All interviews were conducted by the same urologist. And the values of pediatric penileperception score were assessed. Results The mean age was 36 （6 - 144） months. And the mean followup period was 12 （1 - 117） months. The postoperative satisfaction PPPS scores of urologist,parental hypospadias repairing were （2. 83 ± 0. 28）,（2. 32 ± 0. 52） points respectively. For hypospadias repairing,the urologist scoresof penile appearance were markedly higher than those of parents （P 〈 0. 001）. Conclusion PPPS is a reliableinstrument for assessing the postoperative cosmetic outcomes after hypospadias repair. And urologist assessmentis better than that of parents.
