应用泻肺通腑法治疗老年重症肺炎患者的临床研究

Clinical Research of Xiefei Tongfu Decoction on Patients with Severe Acute Pneumonia

目的评价泻肺通腑法在治疗老年重症肺炎合并胃肠功能障碍患者中的作用。方法患者60例随机分为两组,对照组参照2016年《中国急诊重症肺炎临床实践专家共识》治疗,治疗组在指南治疗基础上加用泻肺通腑方药。比较两组患者综合疗效、中医证候疗效及相关指标。结果治疗组在综合疗效、中医证候疗效,改善临床肺部感染评分、急性生理与慢性健康Ⅱ(APACHEⅡ)评分、感染指标、氧合指数等方面优于对照组,能缩短抗生素暴露、机械通气、首次排便时间(P<0.05),但在本研究中住ICU时间及平均住院时间方面未体现明显优势(P>0.05)。结论泻肺通腑方药治疗老年重症肺炎合并胃肠功能障碍疗效明确。

Objective： To evaluate the role of Xiefei TonJu Decoction on elderly patients with severe pnemno- nia and gastrointestinal dysfunction. Methods： The elderly patients with severe pneumonia were randomly divided into two groups. The control group was treated with reference to the consensus on clinical practice of emergency severe pneumonia in China in 2016. The treatment group was treated with Xiefei Tongfu Decoction on the basis of guide therapy. The comprehensive curative eftect of the two groups, the curative eftect of TCM syndrome and the related indexes were compared. Results： The treatment group was superior to the control group in comprehensive cm＇ative effect,TCM syndrome eftect,the improventent of the clinical pulmonary infection scores,the APACHEII scores, the infection index, the oxygenation index and so on,which could shorten the antibiotic exposure, mechani- cal ventilation and the first defecation time （P〈0.05）. However,there was no obvious advantage in ICU time and the average time of hospitalization in this study- （P〉 0.05 ）. Conclusion： The ettect of Xiefei Tongfu Decoction on senile severe pneumonia combined with gastrointestinal dysfunction is clear.
