四君子汤合五苓散加减辨治脾虚水停证肝硬化腹水患者的临床效果

Clinical effect observation of Sijunzi decoction combined with modified Wuling powder in the dialectical treatment of patients with liver cirrhosis and ascites of spleen deficiency water cessation

目的探讨四君子汤合五苓散加减对脾虚水停证肝硬化腹水的治疗效果。方法抽取我院2014年1月至2016年12月接收的90例脾虚水停证肝硬化腹水患者作为研究对象,按数字奇偶法分为对照组和研究组,每组45例。对照组患者给予利尿、加强营养、保肝及维持酸碱、水电解质平衡的常规西医治疗,研究组患者在对照组基础上再给予四君子汤合五苓散加减进行治疗,观察两组患者临床疗效及治疗后肝功能和腹水量积分情况。结果研究组治疗总有效率为95.56%,显著高于对照组的77.78%(P<0.05);治疗后,两组患者腹水量积分明显降低,且研究组显著低于对照组(P<0.05);研究组患者肝功能各指标水平优于对照组,差异具有统计学意义(P<0.05)。结论对脾虚水停证肝硬化腹水患者应用四君子汤合五苓散加减治疗,临床疗效显著,患者肝功能指标明显改善,腹水量积分显著降低。

Objective To explore the effect of Sijunzi decoction combined with modified Wuling powder in the dialectical treatment of patients with liver cirrhosis and ascites of spleen deficiency water cessation.Methods A total of 90 patients with cirrhosis and ascites of spleen deficiency water cessation who treated in our hospital from January 2014 to December 2016 were enrolled in this study.They were divided into control group and study group according to the number parity method,with 45 cases in each group.The control group was treated with diuresis,enhanced nutrition,maitaining balance of acid and alkali,water and electrolyte and other conventional western medicine treatment,while the study group was given Sijunzi decoction combined with modified Wuling powder on the basis of the control group.The clinical efficacy,liver function and ascites amount scores after treatment were observed in the two groups.Results The total effective rate of the study group was 95.56%,which was significantly higher than 77.78%of the control group(P<0.05).After treatment,the scores of ascites in the two groups significantly decreased,and that in the study group was significantly lower than the control group(P<0.05).The levels of liver function indexes in the study group were better than those in the control group,the differences were statistically significant(P<0.05).Conclusion For patients with liver cirrhosis and ascites of spleen deficiency water cessation,Sijunzi decoction and modified Wuling powder has significant clinical effect,it can effectively improve liver function and reduce scores of ascites.
