血清同型半胱氨酸、糖化血红蛋白和血尿酸与2型糖尿病合并冠心病患者冠状动脉病变的相关性分析 被引量:25

摘要 目的研究分析血清同型半胱氨酸(HCY)、糖化血红蛋白(HbA1c)及血尿酸(UA)和2型糖尿病(T2DM)合并冠心病患者冠状动脉病变严重程度的相关性。方法选取2014年2月—2016年2月收治的110例T2DM患者进行研究,按照是否合并冠心病分为观察组(T2DM合并冠心病患者)52例与对照组(单纯T2DM患者)58例。并选取56例同期健康体检者作为健康组。回顾性分析3组的临床资料,检测所选受试者血清HCY、HbA1c及UA水平,研究其与冠状动脉病变严重程度的相关性。结果观察组和对照组HCY、HbA1c、UA水平高于健康组,且观察组高于对照组(P<0.05)。观察组BMI高于对照组和健康组(P<0.05)。多支病变组血清HCY、HbA1c、UA水平明显高于单支和双支病变组(P<0.05)。冠状动脉病变程度与血清HCY、HbA1c、UA水平呈明显正相关性(r=0.462、0.381、0.448,P=0.024、0.041、0.006)。结论 HCY、UA及HbA1c均是T2DM合并冠心病患者发病的危险因素,与冠状动脉病变程度呈正相关关系,对这些指标进行早期识别、合理干预,对改善患者预后和生活质量有重要意义。 A total of 110 T2DM patients admitted during February 2014 and February 2016 were divided into observation group(T2DM combined with coronary heart disease,n=52)and control group(only T2DM,n=58)according to whether or not having coronary heart disease,and other 56 healthy people taking physical examination were selected as healthy group.In 3 groups,clinical data was retrospectively analyzed,and serum levels of homocysteine(HCY),glycosylated hemoglobin(HbA1c)and uric acid(UA)were detected,and then correlations with severe degree of coronary artery disease were studied.Results HCY,HbA1c and UA levels in observation and control groups were significantly higher than those in healthy group,and the levels in observation group were significantly higher than those in control group(P<0.05).Body mass index(BMI)value in observation group was significantly higher than those in control and healthy groups(P<0.05).Serum HCY,HbA1c and UA levels in multiple branches lesions group were significantly higher than those in single and double branches lesions groups(P<0.05).Severe degree of coronary artery disease was positively correlated with serum HCY,HbA1c and UA levels(r=0.462,0.381,0.448,P=0.024,0.041,0.006).Conclusion HCY,UA and HbA1c are risk factors of T2DM complicated by coronary heart disease,and the levels are positively correlated with severe degree of coronary artery disease.Early diagnosis and rational intervention of these indicators are helpful for improving prognosis and life quality of patients with T2DM complicated by coronary artery disease.
