清肺化痰汤治疗重症肺部感染临床疗效及对免疫因子水平的影响

Clinical efficacy of Qingfei Huatan decoction in treating severe pulmonary infection and its influence on immune factors

目的:探讨清肺化痰汤治疗重症肺部感染的临床疗效及对免疫因子的影响。方法:选取重症肺部感染患者86例为研究对象,采用随机数字表分为对照组和观察组各43例,对照组给予注射用头孢哌酮钠舒巴坦钠治疗,观察组在对照组基础上给予自拟清肺化痰汤治疗,比较两组临床疗效及治疗前后免疫因子变化情况。结果:治疗后两组患者中医症状积分均明显下降,且观察组比对照组下降更明显(P<0.05)。两组治疗后WBC、CRP、肺炎指数和APACHEII评分均明显下降,且观察组比对照组更明显(P<0.05)。观察组治疗后治愈率及总有效率均明显高于对照组(P<0.05);两组治疗后CD_3^+T水平均无明显变化(P>0.05),CD_4^+T及CD_4^+T/CD_8^+T水平均显著升高,CD_8^+T水平显著降低,且观察组优于对照组(P<0.05)。对照组治疗后IgM、IgG、IgA无明显变化(P>0.05),观察组治疗后IgM、IgG、IgA水平明显高于治疗前和对照组(P<0.05)。两组患者治疗期间均未出现明显的药物不良反应,两组之间无明显统计学差异(P>0.05)。结论:自拟清肺化痰汤可改善重症肺部感染患者机体免疫功能,临床疗效满意。

Objective:To investigate the clinical efficacy of Qingfei Huatan decoction in treating severe pulmonary infection and its influence on immune factors.Methods:86 cases of severe pulmonary infection were selected and divided into control group and observation group by random number table,43 cases were treated with Cefoperazone Sodium and Sulbactam Sodium for injection,and the control group was given the treatment of Qingfei Huatan decoction on the basis of the control group.compared with the two groups,the clinical effect and immunization before and after treatment were compared.Results:After treatment,the symptom scores of the two groups decreased significantly,and the observation group decreased more significantly than the control group(P<0.05).After treatment,the WBC,CRP,pneumonia index and APACHEII scores of the two groups decreased significantly,and the observation group was more obvious than the control group(P<0.05).After treatment,the cure rate and total effective rate of the observation group were significantly higher than that of the control group(P<0.05).There was no significant change in the level of CD 3+T in the two groups(P>0.05),the average of CD 4+T and CD 4+T/CD 8+T were increased,the level of CD 8+T decreased significantly,and the observation group was superior to the control group(P<0.05).After treatment,there was no significant change in IgM,IgG and IgA in the control group(P>0.05).The levels of IgM,IgG and IgA in the observation group were significantly higher than those in the control group(P<0.05)before treatment.There was no obvious adverse drug reaction in the two groups during the treatment period.There was no significant difference between the two groups(P>0.05).Conclusion:Qingfei Huatan decoction can improve the immune function of patients with severe pulmonary infection.The clinical effect is satisfactory.
