体位改变联合肛管插入深度的改进在婴幼儿水合氯醛灌肠中的效果观察

Improvement of Body Position Change Combined with Anal Canal Insertion Depth in Chloral Enema for Infants

目的探讨实施体位改变联合肛管插入深度的改进在婴幼儿水合氯醛灌肠中的影响。方法便利抽样选择在2016年1月至2017年6月在医院门诊就诊且需作辅助检查的患儿104例,按1∶1随机分为观察组与对照组,观察组采用俯卧位联合增加肛管插入深度行水合氯醛保留灌肠法,对照组按传统保留灌肠法进行灌肠。比较两组患儿镇静起效时间、平均入睡时间、一次性插管的成功率、药物制动效果、Ramsay镇静评分。结果观察组患儿镇静起效时间明显快于对照组、灌肠后平均入睡时间少于对照组、插管的成功率高于对照组、灌肠后药物制动效果及Ramsay评分镇静效果优于对照组,差异均有统计学意义(均P<0.05)。结论采用俯卧位联合增加肛管插入深度的改进行婴幼儿水合氯醛保留灌肠法,实用性强,操作简便,效果优于传统保留灌肠法。

Objective To explore the improvement of position change combined with the depth of anal canal insertion in chloral enema for infants.Methods By convenience sampling method,104 children who were treated in the hospital outpatient department and needed auxiliary examination were selected from January 2016 to June 2017.Patients were randomly divided into observation group and control group,the observation group was treated with chloral hydrate retention enema in prone position combined with increasing the depth of anal canal insertion,while the control group was treated with traditional retention enema.The onset time of sedation,the average sleep time,one-time success rate of tube insertion,drug braking effect,and Ramsay score were compared between two groups.Results The sedation onset time of the observation group was significantly faster than that of the control group;the average sleeping time after enema was less than that of the control group;the success rate of intubation was higher than that of the control group;drug braking effect and Ramsay score sedation effect after enema were better than those of control group,the differences were statistically significant(all P<0.05).Conclusions The modified chloral enema method,which is improved by prone position combined with increasing the depth of anal canal insertion,is more practical and easier to operate,and the effect is better than the traditional retention enema.
