摘要
目的 探讨脾多肽联合二氧化碳(CO2)激光治疗尖锐湿疣的临床效果。方法 研究对象为本科室门诊收治的144例尖锐湿疣患者,将其随机分为观察组和对照组,各72例。对照组CO2激光治疗,观察组脾多肽联合CO2激光治疗,对比分析两组临床治疗效果及治疗前后外周血T细胞亚群的变化。另选择50名健康志愿者为正常组。结果 观察组治愈57例(79.2%),复发15例(20.8%),而对照组治愈40例(55.6%),复发32例(44.4%),两组治愈率和复发率比较,差异均有统计学意义(χ2=9.128、4.267,P=0.003、0.039)。治疗前,144例尖锐湿疣患者CD3+计数、CD4+计数、CD4+/CD8+比值均明显低于正常组(均P<0.05),CD8+计数较正常组高(P<0.05),但观察组和对照组之间差异无统计学意义(均P >0.05)。与治疗前相比,观察组治疗后CD3+计数、CD4+计数、CD4+/CD8+比值升高,CD8+计数降低,差异均有统计学意义(均P<0.05);对照组外周血T细胞亚群治疗前后比较,差异均无统计学意义(均P >0.05)。治疗后,观察组CD3+计数、CD4+计数、CD4+/CD8+比值均高于对照组,CD8+计数较对照组低,差异均有统计学意义(均P<0.05)。结论 脾多肽联合CO2激光治疗尖锐湿疣临床疗效确切,复发率较低,可能与改善患者的细胞免疫功能有关。
Objective To investigate the clinical efficacy of lienal polypeptide combined with CO2 laser in the treatment of condyloma acuminatum and the influence on cellular immune.Methods 144 patients with condyloma acuminatum were included,and randomly divided into an observation group and a control group,72 cases for each group.The control group were treated with CO2 laser,while the observation group with intramuscular lienal polypeptide and CO2 laser.The clinical efficacies and the differences in the distribution of T lymphocyte subgroups in peripheral blood were compared between the two groups.Meanwhile,50 healthy people were chosen as a normal group.Results The cured rate and the recurrence rate were 79.2% and 20.8% in the observation group,and were 55.6% and 44.4% in the control group,respectively,with statistical differences(χ2=9.128 and 4.267,P=0.003 and 0.039).Compared with the normal group,the CD3+,CD4+,and CD4+/CD8+in the 144 patients were lower,while the CD8+was higher,and the differences were statistically significant(all P<0.05).Before the treatment,there were no statistical differences in T-lymphocyte subsets between the observation group and control group.After the treatment,the CD3+,CD4+,and CD4+/CD8+in the observation group markedly increased,while CD8+decreased(all P<0.05).Moreover,the improvement degree of the observation group was much better than that of the control group,with a statisticall difference(P<0.05).Conclusions Lienal polypeptide combined with the CO2 laser in the treatment of condyloma acuminatum has a high efficacy and a low recurrence rate,which may be related to an immune level improvement.
