摘要
目的探讨热休克转录因子1(HSF1)在不同分子分型乳腺癌组织中的表达及其与预后的关系。方法回顾性分析364例乳腺癌患者的临床资料,依据根据雌激素受体(ER)、孕激素受体(PR)和人表皮生长因子受体2(HER2)的表达水平差异,分为Luminal A型组(n=100),Luminal B型组(n=120),HER2型组(n=54),三阴性组(n=90)。采用免疫组织化学染色法检测各组患者乳腺癌组织中HSF1、热休克蛋白70(HSP70)、抗凋亡蛋白B细胞淋巴瘤因子-2(Bcl-2)、促凋亡蛋白Bcl-2相关X蛋白(BAX)、巨噬细胞移动抑制因子(MIF)的表达情况,并分析HSF1的表达与HSP70、Bcl-2、BAX和MIF的相关性,比较HSF1阳性表达与阴性表达患者的5年生存率。结果Luminal A型组和Luminal B型组患者乳腺癌组织中HSF1、HSP70、Bcl-2和MIF的阳性表达率均低于HER2型和三阴性组患者,BAX阳性表达率均高于HER2型组和三阴性组患者(P﹤0.05)。乳腺癌组织中HSF1与HSP70、Bcl-2和MIF的表达呈正相关(P﹤0.05),但与BAX的表达呈负相关(P﹤0.05)。HSF1阴性表达患者的5年生存率为54.24%,明显高于HSF1阳性表达患者的18.29%(P﹤0.01)。结论 HSF1在HER2型和三阴性乳腺癌中的表达水平较高,且HSF1阳性表达患者的预后较差。
Objective To investigate the expression of heat shock transcription factor 1 (HSF1) in breast carcinoma tissues with different molecular subtypes and its relationship with prognosis. Method The clinical data of 364 patients with breast carcinoma were retrospectively analyzed, according to the difference of expression levels of estrogen receptor (ER), progesterone receptor (PR) and human epidermal growth factor receptor 2 (HER2), they were 100 patients of Luminal A breast carcinoma, 120 patients of Luminal B type, 54 patients of HER2 type, and 90 patients of triple-negative type. The expressions of HSF1, heat shock protein 70 (HSP70), anti-apoptotic protein B-cell lymphoma-2 (Bcl-2), pro-apoptotic protein Bcl-2 associated X protein (BAX) and macrophage migration inhibition factor (MIF) were detected by immunohistochemical staining method, and the relationship between the expression of HSF1 and HSP70, Bcl-2, BAX, MIF were analyzed, in addition, the 5-year survival rate of patients with positive and negative expression of HSF1 were compared. Result The positive expression rate of HSF1, HSP70, Bcl-2 and MIF in Luminal A and B breast carcinoma were lower than that in those with HER2 and triple-negative type (P<0.05);the positive expression rates of BAX were higher than those of Luminal A and B type (P<0.05). There was positive correlation between the expression of HSF1 and HSP70, Bcl- 2 and MIF in breast carcinoma tissues (P<0.05), but it was negatively correlated with the expression of BAX (P<0.05). The 5-year survival rate of patients with negative expression of HSF1 was 54.24%, which was significantly higher than that of patients with positive expression of HSF1 at 18.29%(P<0.01). Conclusion HSF1 is highly expressed in HER2 and triple-negative breast carcinoma, and its positive expression is associated with poor prognosis of these patients.
