临床药师依托静脉用药集中调配中心在合理用药中的作用

Role of Clinical Pharmacists Relying on Intravenous Drug Centralized Allocation Center in Rational Drug Use

目的:探讨静脉用药集中调配中心的临床药师在合理用药中的作用。方法:临床药师依托PIVAS,通过纠正临床开具不合理医嘱(主要包括不合理的配伍、不合理的溶媒种类、不合理的溶媒剂量、不合理的药物剂量和不合理的用药频率)、参与指导临床用药和收集药品的不良反应,监测治疗药物的安全性,促进临床合理用药。结果:临床药师通过在PIVAS工作,在提高输液质量的同时,有效减少了临床开具的不合理处方数量,减少了因不合理用药而产生不良反应;参与指导临床用药,有效地避免了护士操作的错误;收集药品的不良反应,充分发挥PIVAS临床药师在药品不良反应检测、评价、分析和反馈中的能动性。结论:临床药师依托PIVAS可提高医院合理用药的水平。

Objective:To discuss the effect of clinical pharmacists in rational use of drugs in Pharmacy Intravenous Admixture Service(PIVAS). Methods:Clinical pharmacists adopt some methods to promote the rational use of drugs,which contain correcting unreasonable medication orders(including unreasonable combination use of drugs,the type and the dose of the solvent,the dose and the frequency of the drugs),guiding the use of the drugs,collecting adverse drug reaction and monitoring the safety of the drugs. Results:By adopting the above methods,clinical pharmacists of the PIVAS have improved the quality of pharmaceutical care,effectively reduced the number of the unreasonable medication orders and the adverse drug reaction of the patients which is produced by unreasonable medication orders,prevented nurses from operation errors,and given full play to the initiative in the detection,evaluation,analysis and feedback of the adverse drug reaction.Conclusion:With the help of PIVAS,the level of the hospital rational use of drugs is more proved.
