拉杆式伤口皮肤扩展器联合封闭负压引流术在小腿慢性创面缺损修复中的应用

Application of skin stretching device combined with closed suction drainage and conventional techniques in the repair of Chronic wound at leg

目的探讨拉杆式伤口皮肤扩展器(Skin stretching device, SSD)联合封闭负压引流技术(Vacuum Sealing Drainage, VSD)在小腿慢性创面缺损修复中的应用及疗效。方法 2017年10月-2018年4月收治小腿慢性创面缺损5例,应用拉杆式伤口皮肤扩展器联合封闭负压引流技术治疗,并观察其术后疗效。结果 5例中3例应用拉杆式伤口皮肤扩展器直接闭合,2例创面较大,无法直接闭合,拉拢皮肤后配合植皮修复创面。5例术后均无感染,术后随访6～18个月,患肢皮肤未形成瘢痕挛缩,相邻关节活动未见受限。结论拉杆式伤口皮肤扩展器联合封闭负压引流技术治疗小腿慢性创面缺损疗效良好,值得临床推广。

Objective To explore the clinical effect of the Skin stretching device combined with closed suction drainage and conventional techniques on chronic wound at leg. Methods From October 2017 to April 2018, 5 cases of chronic wound at leg were admitted to hospital and treated by skin stretching device combined with closed suction drainage and conventional techniques, and curative effects after operation was observed. Results Among 5 patients using Skin stretching device, 3 cases of wound were directly closed, 2 patients had a large wound and could not be closed directly. Skin graft was used to repair the wound. No infection was found in all 5 patients, no scar contracture occurred and unrestricted adjacent joint activities were found in 3-months follow up. Conclusion Using Skin stretching device combined with closed suction drainage and conventional techniques to repair the Chronic wound at leg has good clinical outcomes, which is worthy of promotion.
