无创机械通气在慢性阻塞性肺疾病合并呼吸衰竭患者治疗中的应用效果 被引量:5

Application effect of noninvasive mechanical ventilation in chronic obstructive pulmonary disease patients with respiratory failure

摘要 目的探讨无创机械通气用于慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者的治疗效果。方法选取2016年4月至2017年10月宁波老年康复医院收治的COPD合并呼吸衰竭患者150例,采用随机数字表法分为观察组和对照组,每组75例。对照组采用常规综合治疗方式,观察组在对照组的基础上采取无创机械通气手段。比较两组患者的临床效果。结果观察组治疗后呼吸频率(RR)、心率(HR)和动脉血二氧化碳分压(PaCO2)均优于对照组(t = 13.29、12.48、13.42,均P <0.05),而氧分压(PaCO2)、pH值均明显高于对照组(t=5.91、2.88,均P <0.05)。观察组总有效率为97.33%(73/75),优于对照组的73.33%(55/75),两组差异有统计学意义(x^2=1.81,P<0.05)。结论无创机械通气治疗COPD合并呼吸衰竭效果好,可明显改善患者的呼吸功能,还有效缩短了治疗时间,值得临床推广。 Objective To explore the application effect of noninvasive mechanical ventilation in chronic obstructive pulmonary disease(COPD)patients with respiratory failure.Methods From April 2016 to October 2017,150 COPD patients with respiratory failure in Ningbo Geriatric Rehabilitation Hospital were selected,and they were divided into observation group and control group by using the random number table method,with 75 cases in each group.The control group was treated with conventional comprehensive therapy,the observation group was treated with noninvasive mechanical ventilation method on the basis of the control group.The clinical effect of the two groups was compared.Results In the observation group,the RR,HR and arterial blood PaCO2 were significantly better than those in the control group(t=13.29,12.48,13.42,all P<0.05).The PaO2,pH value in the observation group were significantly higher than those in the control group(t=5.91,2.88,all P<0.05).The total effective rate of the observation group was 97.33%(73/75),which was higher than 73.33%(55/75)of the control group,the difference was statistically significant(X^2=1.81,P<0.05).Conclusion Application of noninvasive mechanical ventilation in COPD patients with respiratory failure can significantly improve respiratory function,and not only effectively shorten the treatment time,but also improve the treatment effect,and it is worthy of clinical promotion.
