Objective To explore the effect of health education model with knowledge lecture-post-discharge follow-up visit on self-efficacy in patients with type 2 diabetes. Methods A total of 110 hospitalized patients with diabetes from January to December in 2017 were collected and randomly divided into the control group and the experimental group, with 55 cases in each group. The control group was mainly given traditional routine nursing care, and the corresponding health education and treatment were carried out for the patients. In addition to the routine care and treatment, the experimental group was also given joint knowledge lectures and post-discharge follow-up visit for patients. The differences in self-efficacy, fasting blood glucose, and glycosylated hemoglobin levels between the two groups were observed at admission and 3 months after discharge. Results 3 months after discharge, the experimental group, which was intervened by knowledge lecture-post-discharge follow-up visit, was higher in the self-efficacy score than the control group(P<0.05);the improvement of fasting blood glucose and glycosylated hemoglobin was also superior to the control group(P<0.05). Conclusion The health management model of knowledge lecture - post-discharge follow-up visit can improve the self-management behavior of diabetic patients, enhance confidence, improve blood sugar, and benefit patients.
