尖锐湿疣患者人乳头状瘤病毒感染、体液免疫状态与复发的关系

The relationship between HPV infection, humoral immune status and recurrence in patients with condyloma acuminatum

目的探讨尖锐湿疣(CA)患者人乳头状瘤病毒(HPV)感染、体液免疫状态与复发的关系。方法选取2016年1月-2018年3月本院CA患者200例,依据HPV分型分为高危组(n=50)和低危组(n=150),随访并观察3个月内复发情况,同期选取健康人员50例作为对照组,所有人员均检测体液免疫状态[免疫球蛋白(Ig)A、IgG、IgM],分析CA患者HPV感染、体液免疫状态与复发的关系。结果高危组IgA、IgG、IgM明显低于低危组,低危组IgA、IgG、IgM明显低于对照组(P<0.05);200例CA患者中,3个月内复发76例、复发率为38.00%,复发者高危HPV感染率明显高于无复发者,复发者低危HPV感染率、IgA、IgG、IgM明显低于无复发者(P<0.05)。结论 CA患者HPV感染可引起机体体液免疫功能下降且高危HPV感染更严重,而HPV感染、体液免疫状态与CA复发有关,其中高危HPV感染及体液免疫功能下降是CA复发的重要原因。

Objective To discuss the relationship between human papilloma virus(HPV) infection,humoral immune status and recurrence in patients with condyloma acuminatum(CA).Methods A total of 200 patients with CA were selected from January 2016 to March 2018 in our hospital.According to HPV classification,the patients were divided into high risk group(n=50) and low risk group(n=150),the recurrence was followed up and observed within 3 months,50 healthy persons were selected as the healthy group.The humoral immune status [immunoglobulin(Ig) A,IgG,IgM] were tested in all persons.The relationship between HPV infection,humoral immune status and recurrence in patients with CA was analyzed.Results IgA,IgG,IgM in high-risk group were significantly lower than those in low-risk group,and that in low-risk group were significantly lower than those in control group,with the differences statistically significant(P<0.05).In 200 patients with CA,76 cases recurred within 3 months,and the recurrence rate was 38.00%,the high-risk HPV infection rate in recurrence patients was significantly higher than that in non-recurrence patients,and the low-risk HPV infection rate,IgA,IgG,IgM in recurrence patients were significantly lower than those in non-recurrence patients(P<0.05).Conclusion HPV infection in CA patients can cause the decline of humoral immune function,and the high-risk HPV infection is more serious.HPV infection and humoral immune status are related to the recurrence of CA,and high-risk HPV infection and the decline of humoral immune function are important reasons for the recurrence of CA.
