儿童手足口病合并细菌感染患者PCT水平早期检测的临床分析 被引量:2

摘要 目的探讨手足口病患儿合并细菌感染患者PCT水平早期检测的临床意义。方法选择2016年1月至2017年1月本院接诊的231例手足口病患儿作为研究对象。同时,选随机取本院体检中心20例健康儿童作为对照组。根据检查结果将研究组231例患儿分为合并细菌感染组(55例)以及单纯病毒感染组(176例)。分析比较外周血WBC计数及血清CRP、PCT水平。结果3组外周血WBC计数及血清CRP、PCT水平比较,差异具有统计学意义(F=26.56、24.39、382.32,均P<0.05);3组外周血WBC水平以及血清CRP、PCT阳性结果检出率比较,差异具有统计学意义(P<0.05),其中合并细菌感染组的外周血WBC水平以及血清CRP、PCT水平阳性率明显高于对照组和单纯病毒感染组;外周血WBC计数及血清CRP、PCT水平的敏感度分别为87.61%、85.86%、93.12%,特异度分别为94.00%、91.00%、95.00%。结论血清PTC水平检测有特异性高、准确性好的特点,且检查方便快捷、结果有效,可以作为鉴别手足口病患儿是否存在细菌感染的敏感指标,值得在临床上推广。 Objective To investigate the clinical significance of early detection of PCT in children with hand, foot and mouth disease (HFMD) complicated with bacterial infection. Methods 231 children with HFMD treated at our hospital from January, 2016 to January, 2017 were selected as study objects, and were divided into a bacterial infection group (55 cases) and a simple virus infection group (176 cases) according to the detection results. 20 healthy children were selected from the physical examination center of our hospital as a control group. The peripheral blood WBC count and serum CRP and PCT were analyzed and compared. Results There were statistical difference in WBC in peripheral blood and serum CRP and PCT levels among the 3 groups (F = 26.56, 24.39, and 382.32, all P < 0.05). There were statistical difference in the detection rate of WBC in peripheral blood and serum CRP and PCT positive results among the 3 groups (P < 0.05);the level of WBC in peripheral blood and the positive rates of serum CRP and PCT were significantly higher in the bacterial infection group than in the control group and the simple virus infection group. The sensitivities of peripheral blood WBC count and serum CRP and PCT were 87.61%, 85.86% and 93.12%, and the specificities were 94.00%, 91.00%, 95.00%. Conclusion Detection of serum PTC have high specificity, good accuracy, and convenient examination results, is effective, and can be used as identification of children with HFMD whether there are sensitive indicators of bacterial infection, so it is worthy of clinical promotion.
