JCI标准结合标准化急救流程用于儿科急诊急救的时效性及安全性探讨

Discussion on the Timeliness and Safety of JCI Standard Combined with Standardized First Aid Procedure in Pediatric Emergency Emergency

目的:探讨美国医疗机构评审国际联合委员会(JCI)标准结合标准化急救流程用于儿科急诊急救过程中时效性及安全性改善作用.方法:选取本院2016年6月-2018年5月的儿科急诊急救患儿98例,依据实施护理管理模式不同分为对照组(n=50)与观察组(n=48).对照组于2016年6月-2017年5月仅实施JCI标准,观察组于2017年6月-2018年5月在JCI标准的基础上结合标准化急救流程模式,对比两组抢救时效性及安全性.结果:观察组急诊等候时间(20.43±2.17)min、抢救室滞留时间(0.58±0.12)h、抢救医嘱执行时间(24.19±5.26)s,均明显短于对照组(25.42±3.26)min、(1.72±0.35)h、(42.49±10.23)s,差异均有统计学意义(P<0.05);观察组药品相关风险事件发生率(2.08%)、仪器相关风险事件发生率(0)、抢救室滞留率(6.25%)、护患相关不良事件发生率(4.17%)均明显低于对照组的14.00%、10.00%、28.00%、20.00%,差异均有统计学意义(P<0.05);观察组药品摆放合格率(100%)与抢救成功率(100%)均明显高于对照组的76.00%、84.00%,差异均有统计学意义(P<0.05).结论:在JCI标准的基础上结合标准化急救流程用于儿科急诊急救过程中,能够缩短患儿候诊时间、缩短医嘱执行时间,降低因药品、仪器、人员造成的护理风险,减少抢救室滞留,有助于降低护理不良事件发生风险,且抢救成功率高,其时效性及安全性均佳.

Objective:To investigate the efficacy and safety improvement of joint international commission on accreditation of American medical institutions(JCI)standard combined with standardized first aid procedure in pediatric emergency treatment.Method:98 pediatric emergency children from June 2016 to May 2018 in our hospital were selected and divided into control group(n=50)and observation group(n=48)according to the different mode of implementing nursing management.The control group only implemented the JCI standard from June 2016 to May 2017,while the observation group combined the standardized first aid procedure based on the JCI standard from June 2017 to May 2018.The effectiveness and safety of rescue were compared between the two groups. Result:The emergency waiting time(20.43±2.17)min,the stay time of the rescue room(0.58±0.12)h, the execution time of doctor’s orders(24.19±5.26)s in the observation group were significantly shorter than (25.42±3.26)min,(1.72±0.35)h,(42.49±10.23)s in the control group,the differences were statistically significant(P<0.05).The incidence of drug-related risk events(2.08%),the incidence of instrument-related risk events(0),the retention rate in the emergency room(6.25%),and the incidence of nurse-patient related adverse events(4.17%)in the observation group were significantly lower than 14.00%,10.00%,28.00%, 20.00% of the control group,the differences were statistically significant(P<0.05).The qualified rate of drug placement(100%)and the success rate of rescue(100%)were significantly higher than 76.00%,84.00% of the control group,the differences were statistically significant(P<0.05).Conclusion:On the basis of JCI standard combine with standardized first aid procedure in pediatric emergency treatment,it can reduce the waiting time of children,the execution time of doctor’s orders,the nursing risk caused by drugs,instruments and personnel, and the stay time of emergency room,it helps to reduce the risk of adverse nursing events,and the success rate of rescue is high,its timeliness and safety are good.
