超声多模式技术在鉴别诊断甲状腺结节良恶性中的应用价值 被引量:5

摘要 目的探讨超声多模式技术在鉴别诊断甲状腺结节良恶性中的应用价值。方法方便选择2017年1月-2018年12月期间于该院就诊的92例甲状腺结节患者为研究对象,所有患者均接受普通超声、超声弹性成像、超声造影及超声引导下细针穿刺活检等检查,并以最终手术病理检查为金标准,评估各检查结果。结果普通超声检出阳性40例,阴性52例,真阳31例,真阴25例,诊断敏感性、特异性及准确性为53.4%、73.5%、60.9%;普通超声+弹性成像检出阳性50例,阴性42例,其中真阳43例,真阴27例,诊断敏感性、特异性及准确性为74.1%、79.4%、76.1%;普通超声+弹性成像+超声检出阳性52例,阴性40例,其中真阳46例,真阴28例,诊断敏感性、特异性及准确性为79.3%、82.4%、80.4%;超声多模式技术检出阳性57例,阴性35例,其中真阳57例,真阴34例,诊断敏感性、特异性及准确性为98.3%、100.0%、98.9%;超声各模式检查诊断敏感性、特异性及准确性均低于超声多模式技术联合检查,差异有统计学意义(χ^2=32.673、9.761、40.911,P=0.000、0.021、0.000)。结论超声多模式技术在鉴别诊断甲状腺结节良恶性中具有极高的应用价值,利于提高临床诊断的准确性。 Objective To investigate the value of ultrasound multi-mode technique in the differential diagnosis of benign and malignant thyroid nodules. Methods A total of 92 patients with thyroid nodules who were admitted to our hospital from January 2017 to December 2018 were convenient selected. All patients underwent general ultrasound, ultrasound elastography, contrast-enhanced ultrasound and ultrasound-guided fine needle biopsy. And the final surgical pathological examination as the gold standard, evaluate the results of each test. Results 40 cases were positive by ordinary ultrasound, 52 cases were negative, 31 cases were true yang, 25 cases were true yin. The diagnostic sensitivity, specificity and accuracy were53.4%, 73.5%, 60.9%;common ultrasound + elastography was positive. There were 50 cases and 42 cases were negative,including 43 cases of true yang and 27 cases of true yin. The diagnostic sensitivity, specificity and accuracy were 74.1%,79.4%, and 76.1%;52 cases were detected by ordinary ultrasound + elastography + ultrasound. There were 40 cases of negative, including 46 cases of true yang and 28 cases of true yin. The diagnostic sensitivity, specificity and accuracy were79.3%, 82.4%, and 80.4%. 57 cases were positive by ultrasound multi-mode technique and 35 cases were negative. 57 cases of yang, 34 cases of true yin, the diagnostic sensitivity, specificity and accuracy were 98.3%, 100.0%, 98.9%;the sensitivity, specificity and accuracy of ultrasound mode diagnosis were lower than the ultrasound multi-modal technique combined examination The difference was statistically significant(χ^2=32.673, 9.761, 40.911, P=0.000, 0.021, 0.000). Conclusion Ultrasound multi-mode technique has a high application value in the differential diagnosis of benign and malignant thyroid nodules, which is conducive to improving the accuracy of clinical diagnosis.
