肺结节加强观察功能对双源CT低剂量扫描图像肺结节检出效能的影响

Effect of enhanced observation function of pulmonary nodules on detection efficiency of pulmonary nodules in low-dose dual-source CT images

摘要 目的探索肺结节加强观察功能(NEV)在双源CT低剂量扫描图像肺结节检出效能中的影响力.方法回顾性收集2017年3月至2018年7月在我院接受胸部双源CT低剂量扫描患者的临床资料,由同一组医师采用常规方法阅片和采用辅助NEV分析软件阅片,观察2种方法的肺结节检出率、阅片时间等.结果放射科医师阅片的实性结节检出率为66.9%,对<0.5 cm结节的检出率为69.2%,总检出率为76.3%;医师辅助NEV阅片的实性结节检出率为94.8%,<0.5 cm结节检出率为96.4%,总检出率为95.8%,经对比差异均有统计学意义(P<0.05);对于磨玻璃样结节检出率和0.5~2.0 cm结节检出率:2种阅片方法之间差异无统计学意义,P>0.05;阅片时间比较:放射科医师阅片为(168±24)s/例,医师辅助NEV阅片为(218±32)s/例,差异有统计学意义(P<0.05).结论双源CT低剂量扫描图像的阅片中,放射科医师辅助NEV功能阅片虽然会延长阅片时间,但是能显著提高肺结节的检出率,尤其是体积微小的结节和实性结节的检出率,值得推广. Objective To explore the influence of enhanced observational function(NEV) of pulmonary nodules on the detection efficiency of low-dose dual-source CT images of pulmonary nodules. Methods The clinical data of patients who received low-dose chest dual-source CT scans in our hospital from March 2017 to July 2018 were retrospectively collected. The same doctors used routine methods to read films and assisted NEVanalysis software respectively to observe the detection rate of pulmonary nodules and the time of reading films. Results The detection rate of solid nodules was 66.9%.the detection rate of nodules less than 0.5 cm was 69.2%, the total detection rate was 76.3%;while the detection rate of solid nodules by NEV films assisted by doctors was 94.8%,the detection rate of nodules less than 0.5 cm was 96.4%, and the total detection rate was 95.8%,There were significant differences between the two groups(P<0.05). For ground glass nodules and 0.5~2.0 cm nodules, there was no significant difference between the two methods(P>0.05);the time of reading was(168±24) s/case for radiologists and(218±32) s/case for NEV assisted by doctors(P< 0.05). Conclusion Although radiologists assisted NEV functional film reading can prolong the reading time, it can significantly improve the detection rate of pulmonary nodules,especially small nodules and solid nodules,which is worthy of promotion.
