多次法根管治疗与一次性根管治疗牙体牙髓病临床效果评价研究

Evaluation of Clinical Effects of Multiple Root Canal Therapy and One-time Root Canal Therapy for Dental Pulp Disease

目的牙体牙髓病患者应用多次法根管及一次性的根管治疗效果情况分析。方法选取2018年1月—2019年1月该院诊治70例牙体牙髓病患者资料,按照随机数表分两组,对照组予多次根管治疗,研究组予一次性的根管治疗方案,分析两组患者咀嚼功能、临床疗效及并发症情况。结果治疗后,研究组牙龈指数(0.35±0.08)、出血指数(0.38±0.06)比对照组低,且咬合力(138.03±15.46)lbs及咀嚼效率(93.52±8.35)%等咀嚼指标比对照组高,比较差异有统计学意义(t=13.536 8、14.048 7、6.543 2、8.549 2,P<0.05);研究组显效24例,有效10例,研究组临床总有效97.14%比对照组77.14%高,比较差异有统计学意义(χ^2=4.590 2,P<0.05);且研究组发生咬合不适、牙龈痛、牙龈肿胀等并发症5.71%比对照组22.86%少,比较差异有统计学意义(χ^2=4.200 0,P<0.05)。结论牙体牙髓病患者应用一次性相关根管治疗,相比多次法根管,更能有效改善患者咀嚼功能指标,还能减少并发症发生率,临床效果显著,具一定临床应用价值。

Objective To analyze the effect of multiple root canals and one-time root canal treatment in patients with dental pulp disease. Methods The data of 70 patients with dental pulp disease were treated in our hospital from January 2018 to January 2019. The patients were divided into two groups according to the random number table. The control group received multiple root canal treatments. The study group gave a one-time root canal. The treatment plan was used to analyze the chewing function, clinical efficacy and complications of the two groups. Results After treatment, the gingival index (0.35±0.08) and bleeding index (0.38±0.06) were lower in the study group than in the control group, and the bite force (138.03±15.46) lbs and chewing efficiency (93.52±8.35)% were compared with the control index. The difference was statistically significant (t=13.536 8、14.048 7、6.543 2、8.549 2,P<0.05);the study group was effective in 24 cases and effective in 10 cases. The clinical total effective rate of the study group was 97.14% higher than that of the control group (77.14%). the difference was statistically significant (χ^2=4.590 2,P<0.05);and the study group had 5.71% of complications such as occlusion discomfort, gingival pain and gum swelling, which was less than 22.86% of the control group. The difference was statistically significant(χ^2=4.200 0,P<0.05). Conclusion One-time related root canal therapy is used in patients with dental pulp disease. Compared with multiple root canals, it can effectively improve the chewing function index and reduce the incidence of complications. The clinical effect is significant and has certain clinical application value.
