鲍严钟治疗特发性少弱精子症用药规律分析研究

An Analysis of Administration Rules of BAO Yanzhong in Treating Idiopathic Oligospermia

目的:挖掘分析鲍严钟主任中医师临床治疗男性特发性少弱精子症的用药特点和规律。方法:收集2014年6月—2017年9月期间鲍主任治疗男性特发性少弱精子症200例的处方共1008张,采用频率、频数以及指标聚类分析方法进行研究分析。结果:其中45味药使用频率>10%,前10味依次为枳壳、牛膝、甘草、当归、菟丝子、女贞子、淫羊藿、红花、补骨脂、胡芦巴。聚类分析法将药物聚共有八类:益肾填精、温阳补肾、补气养血、养阴生津、清热解毒、理气止痛、健脾燥湿、活血通络等药。结论:鲍主任治疗弱精子症处方中常用药物可归纳为8大类,45味;在辨证的基础上,其处方用药或补肾与益气养血合用,或健脾利湿与活血化瘀合用,或兼以益肾通络,推崇灵活加减,随症治疗。

Objective: To summarize and analyze the administration rules of BAO Yanzhong in treating male idiopathic oligospermia. Methods:A total of 1008 prescriptions of 200 cases of idiopathicoligospermia in men treated by Director BAO from June2014 to September 2017 were collected. Frequency analysis and cluster analysis were adopted for research. Results:Among all herbs,45 kinds were used with a frequency of over 10%. The top ten Chinese herbs were FructusAurantii,Radix AchyranthisBidentatae,Radix etRhizomaGlycyrrhizae, Radix AngelicaeSinensis, Semen Cuscutae, FructusLigustriLucidi, HerbaEpimedii, FlosCarthami,FructusPsoraleae and Semen Trigonellae. Through cluster analysis,the herbs were grouped into eight categories:kidney-tonifying and essence-supplementing, yang-warming and kidney-tonifying, qi-tonifying and blood-nourishing, yin-nourishingand jin-promoting,heat-clearing and detoxifying,qi-regulating and pain-relieving,spleen-invigorating and dampness-eliminating,and blood-activating and collaterals-clearing. Conclusion:The commonly-used herbs in his prescriptions can be classified into eight categories with a total of 45 kinds.Based on syndrome differentiation,Director BAO prescribes herbs usually with the combination of kidney-tonifying,qi-tonifying and blood-nourishing, or with the combination of spleen-invigorating and dampness-eliminating, and blood circulation-promoting and blood stasis-removing,or with the combination of kidney-tonifying and collaterals-unblocking, and he flexibly modifies the herbs to treat oligospermia.
