关节镜下前方入路联合后内入路治疗腘窝囊肿的临床疗效

Clinical efficacy of arthroscopic treatment for popliteal cysts with anterior approach and posteromedial approachs

目的探讨膝关节镜下前方入路联合后内入路治疗腘窝囊肿的临床疗效。方法选取2013年1月-2017年8月收治的100例腘窝囊肿手术患者,根据手术类型分为3组。A组37例,采用关节镜下前方入路联合后内入路手术治疗,其中男17例,女20例,年龄(54.49±8.32)岁;B组30例,采用关节镜手术联合后路开放手术治疗,其中男12例,女18例,年龄(54.23±8.21)岁;C组33例,采用传统后路开放手术治疗,其中男16例,女17例,年龄(53.70±10.11)岁;记录手术前后视觉模拟评分(VAS)、Lysholm功能评分、Rauschning和Lindgren分级、手术时间、切口长度、切口愈合情况、住院天数和囊肿复发情况等。结果A组在术后VAS评分、住院天数、术后Lysholm评分、伤口愈合率和切口长度等均优于B组和C组;A组和B组复发率低于C组;A组和C组手术时间明显低于B组。结论关节镜下前方入路联合后内入路治疗腘窝囊肿是一种值得临床推广的方法。

Objective To assess the clinical efficacy of arthroscopic treatment for popliteal cysts with anterior approach and posteromedial approaches.Methods From January 2013 to August 2018,100 patients with popliteal cysts were assigned into three groups according to different surgery methods.Group A,included 37 patients[17 males and 20 females,mean age(54.49±8.32)years],underwent arthroscopic resection with anterior approach and posteromedial approaches;Group B,included 30 patients[12 males and 18 females,mean age(54.23±8.21)years],underwent open excision after arthroscopic treatment;Group C,included 33 patients[16 males and 17 females,mean age(53.7±10.11)years],underwent open excision.All the patients were estimated by follow-up.The following parameters were compared:the visual analog scale(VAS)for pain,the Lysholm score,Rauschning and Lindgren classification,the time of surgery,the length of incision,the incision healing rate,the hospitalization time,the recurrence rate.Results Group A exhibited better outcomes compared to group B and C in the the postoperative VAS score,the hospitalization time,the postoperative Lysholm score,the incision healing rate,length of incision.The recurrence rate in group A and B is lower than group C;The time of surgery is significantly lower in group A and C than group B.Conclusion Arthroscopic treatment for popliteal cysts with anterior approach and posteromedial approaches is a method worthy of clinical promotion.
