摘要
目的研究椎管内肿瘤患者实施半椎板切除显微手术的临床效果。方法方便选择于该院接受椎管内肿瘤治疗的患者72例,纳入时间为2017年2月-2018年2月,采用随机数字表法将其分为实验组与参照组,每组均36例。其中参照组采用全椎板切除术,实验组行半椎板切除显微手术,对比两组患者手术相关指标术后症状评分不良反应。结果实验组手术时长、住院时长均短参照组,与参照组相比,实验组出血量较少(t=42.634 4、11.149 9、50.937 1,P<0.05),实验组肌肉萎缩、颈部疼痛、运动障碍、麻木等症状评分均低于参照组(t=3.869 3、2.480 1、3.464 4、2.771 9,P=0.000 2、0.015 5、0.000 9、0.007 1),实验组脑脊液漏、感染、肌肉乏力等椎管内肿瘤术后不良反应发生率(5.56%)高于参照组(27.78%),两者差异有统计学意义(χ2=4.180 6,P=0.040 8 <0.05)。结论椎管内肿瘤患者应用半椎板切除显微手术方式效果显著,患者症状得以显著缓解,且手术安全性水平较高。
Objective To study the clinical effect of microsurgery for semi-laminectomy in patients with intraspinal tumors. Methods Convenient selected seventy-two patients who underwent treatment for intraspinal tumors in this hospital were enrolled. The enrollment time was from February 2017 to February 2018. They were divided into experimental group and reference group by random number table method, with 36 cases in each group. The reference group was treated with total laminectomy, and the experimental group underwent micro-surgical microsurgery. The adverse reactions of postoperative symptom scores were compared between the two groups. Results The length of the operation and the length of hospital stay in the experimental group were shorter than the reference group. Compared with the reference group, the experimental group had less bleeding (t=42.634 4, 11.149 9,50.937 1, P<0.05), muscle atrophy, neck pain, exercise in the experimental group. Symptoms such as obstacles and numbness were lower than the reference group (t=3.869 3, 2.480 1, 3.464 4, 2.771 9, P=0.000 2, 0.015 5, 0.000 9, 0.007 1). The experimental group had cerebrospinal fluid leakage, infection, muscle weakness and other intraspinal tumors. The reaction rate (5.56%) was higher than that of the reference group (27.78%), and the difference was statistically significant(χ2=4.180 6, P=0.040 8 <0.05). Conclusion The use of semi-laminar microsurgery in patients with intraspinal tumors is effective, the symptoms of patients are significantly relieved, and the level of surgical safety is high.
