儿童1型糖尿病持续皮下胰岛素输注的效果分析

Effect of continuous subcutaneous insulin infusion in children with type 1 diabetes mellitus

目的探讨1型糖尿病患儿采取持续皮下胰岛素输注治疗的临床疗效。方法随机选取本院2015年4月至2018年4月收治的82例1型糖尿病患儿,并将其均分为对照组(n=41)与研究组(n=41)。对照组患儿采取胰岛素多次皮下注射方案治疗,研究组患儿则采取胰岛素持续皮下输注方案治疗,对比分析两组治疗效果。结果治疗后,两组患儿FPG、2hPG、HbA1c、MAGE、MODD及SDBG指标水平低于治疗前,差异有统计学意义(P<0.05);治疗后,两组患儿组间FPG、2hPG、HBA1c指标水平比较,差异无统计学意义;研究组患儿MAGE(24h内平均血糖波动幅度)、MODD(日间血糖平均绝对差)及SDBG(全天血糖值标准差)均明显低于对照组,差异有统计学意义(P<0.05);两组均无严重低血糖情况发生,低血糖发生率组间比较,研究组低于对照组,差异有统计学意义(P<0.05)。结论对1型糖尿病患儿采取持续皮下胰岛素输注治疗,疗效和安全性显著,值得在临床推广。

Objective To investigate the clinical effect of continuous subcutaneous insulin infusion in children with type 1 diabetes mellitus. Methods 82 children with type 1 diabetes admitted to our hospital from April 2015 to April 2018 were randomly selected and divided into control group (n=41) and study group (n=41).The control group was treated with multiple subcutaneous insulin injections,while the study group was treated with continuous subcutaneous insulin infusion.The therapeutic effects of the two groups were compared and analyzed.Results After treatment,the levels of FPG,2 hPG,HbA1c,MAGE,MODD and SDBG in the two groups were lower than those before treatment,with significant difference (P< 0.05);After treatment,there was no significant difference in the levels of FPG,2 hPG and HBA1c between the two groups.In the study group,the levels of MAGE (mean fluctuation of blood sugar within 24 hours),MODD (mean absolute difference of blood sugar between days) and SDBG (standard deviation of blood sugar value throughout the day) were significantly lower than in the control group,the difference was statistically significant (P<0.05).There was no serious hypoglycemia in both groups,the incidence of hypoglycemia in the study group was lower than that in the control group (P<0.05).Conclusion Continuous subcutaneous insulin infusion is effective and safe for children with type 1 diabetes mellitus,which is worthy of clinical promotion.
