摘要
目的 探讨肾上腺素递增剂量方程G =(K +2 n -1)mg/3min在心肺复苏中的应用效果及临床价值。方法 把114例心跳骤停 (CA)患者分成 3组 ,即标准肾上腺素剂量对照组 (SDE对照组 ) 5 6例 ,方程G =(K +2 n -1)mg/3min(K =1、2 ,n =1、2、3、4… )中首次剂量K =1mg方程 1组 2 6例及K =2mg方程 2组 32例 ,采用肘静脉通道按下列方法应用肾上腺素 :①SDE对照组首次静推肾上腺素 1mg后无效 ,每间隔 3min重复首次剂量 ;②方程 1组首次肾上腺素 1mg无效后 ,每间隔 3min按方程计算出的递增剂量为 2、3、5、9mg……快速静推 1次 ;③方程 2组首次剂量为 2mg无效后 ,按方程计算出来的 3、4、6、10mg……肾上腺素每间隔 3min快速静推 1次 ,同时进行心电、平均动脉压 (MAP)、心率 (HR)自主循环恢复的时间等监测 ,并进行复苏效果评价。结果 ①方程 2组和方程 1组自主循环恢复率、2 4h存活率、出院存活率、出院存活Glasgow昏迷评分与SDE对照组比较均显著升高 (P <0 0 1) ,方程 2组和方程 1组从CPR始用药到 +ROSC平均时间与SDE对照组相比显著缩短 (P<0 0 1)。②方程 2组、方程 1组和SDE对照组从CPR始至 +ROSC中所用肾上腺素的平均用量无明显差异 (P >0 0 5 ) ,但方程 2组和方程 1组达到
