雌二醇对失血性休克家兔肝肾的保护作用

摘要 目的通过检测失血性休克家兔肠系膜微循环、肝肾组织中IL-6表达、CD68巨噬细胞浸润情况以及血清中IL-6,IL-8含量的变化,探讨雌激素对失血性休克家兔肠系膜微循环的影响以及对肝、肾重要器官的保护作用机制.方法将家兔随机分为5组,即模型组(MD组)、生理盐水治疗组(NC组)、雌二醇治疗组(ET组)、山莨菪碱治疗组(AN组)和雌二醇预防组(EP组).采用Chaudry方法复制家兔失血性休克模型,并记录肠系膜微循环变化,分别于休克前、休克Ⅱ期1 h、治疗后2 h采血,分离血清,备用.在预定时间段取肝、肾组织,石蜡包埋,HE切片,行光学显微镜观察,通过免疫组化法检测家兔肝、肾组织中IL-6,CD68的表达情况,酶联免疫吸附法(ELISA)测定血清中IL-6,IL-8浓度的变化.结果休克治疗2 h后,雌二醇预防组、山莨菪碱治疗组的微循环明显优于其他组;HE染色发现雌二醇预防组、山莨菪碱治疗组肝、肾组织损伤程度较生理盐水治疗组、模型组损伤程度轻;雌二醇预防组和山莨菪碱治疗组肝、肾组织IL-6阳性表达率和CD68阳性巨噬细胞浸润程度低于其他组;血清学检查休克Ⅱ期1 h、雌二醇预防组血清IL-6,IL-8浓度低于其他组,差异具有统计学意义(P<0.05);治疗后2 h,雌二醇预防组和山莨菪碱治疗组血清中IL-6和IL-8浓度低于其他组,差异具有统计学意义(P<0.05).结论雌激素可改善失血性休克家兔肠系膜的微循环,对肝、肾具有一定的保护作用. Objective The effect of estrogen on mesenteric microcirculation in rabbits with hemorrhagic shock and the protective mechanism of liver and kidney organs were investigated by detecting the expression of IL-6 and the infiltration of CD68 macrophages in liver and kidney tissues as well as the content of IL-6 and IL-8 in serum.Method The rabbits were randomly divided into 5 groups,namely Model group(MD group),Normal saline treatment group(NC group),Estradiol treatment group(ET group),Anisodamine treatment group(AN group)and Estradiol prevention group(EP group).The hemorrhagic shock model was replicated by Chaudry method,and the changes of mesenteric microcirculation were recorded respectively before the shock,1 h shock periodⅡ,blood sampling after 2 h treatment,separation of serum,set aside.Liver and kidney tissues were collected and embedded at a predetermined time,and HE sections were observed by optical microscope.The expression of IL-6 and CD68 in liver and kidney tissues of rabbits were detected by immunohistochemistry,the changes in serum IL-6 and IL-8 concentrations were determined by ELISA.Results After 2 h of shock treatment,the changes of microcirculation in the Estradiol prevention group and the Anisodamine treatment group were significantly better than those in other groups.HE staining showed that the liver and kidney tissues of the Estradiol prevention group and the Anisodamine treatment group were less damaged than those of the Normal Saline treatment group and the model group.The IL-6 positive expression rate and CD68 positive macrophage infiltration in liver and kidney tissues of the Estradiol prevention group and the Anisodamine treatment group were lower than those of the other groups.ShockⅡserologic testing period 1 h,IL-6,IL-8 serum concentration of Estradiol prevention group is lower than other groups,with statistical significance(P<0.05);Two hours after treatment,serum concentrations of IL-6 and IL-8 in the Estradiol prevention group and the Anisodamine treatment group were lower than those in other groups,with statistical significance(P<0.05).Conclusion Estrogen can improve mesenteric microcirculation and protect liver and kidney in rabbits with hemorrhagic shock.
