颈动脉稳定与不稳定斑块急性脑卒中患者UA、LP-PLA2、D-D、Hcy、Fbg水平对照研究

Comparative study on serum UA,Lp-PLA2,D-D,Hcy and Fbg levels of acute stroke patients with carotid artery stable and unstable plaques

目的探讨颈动脉稳定与不稳定斑块急性脑卒中患者血尿酸(UA)、血浆脂蛋白磷脂酶A2(Lp-PLA2)、D-二聚体(D-D)、同型半胱氨酸(Hcy)、纤维蛋白原(Fbg)水平对照情况。方法选取广州医科大学附属第六医院2017年7月~2019年2月收治的75例颈动脉不稳定斑块急性脑卒中患者作为观察组,选取同期颈动脉稳定斑块急性脑卒中患者75例作为对照组。观察两组患者的UA、Lp-PLA2、D-D、Hcy、Fbg表达水平情况,观察颈动脉不稳定斑块急性脑卒中患者斑块指数和UA、Lp-PLA2、D-D、Hcy、Fbg相关性情况,观察颈动脉不稳定斑块急性脑卒中患者斑块指数多因素回归分析情况。结果观察组患者的UA、Lp-PLA2、D-D、Hcy、Fbg表达水平均高于对照组,颈动脉不稳定斑块急性脑卒中患者斑块指数和UA、Lp-PLA2、D-D、Hcy、Fbg均呈现明显正相关,以斑块指数作为因变量,UA、Lp-PLA2、D-D、Hcy、Fbg作为自变量,建立回归方程:Y=7.675+0.058X1+0.086X2+0.092X3+0.027X4+0.048X5,多元性逐步回归方程有统计学位意义(F=85.437,P=0.000)。结论颈动脉不稳定斑块急性脑卒中患者UA、Lp-PLA2、D-D、Hcy、Fbg表达水平和斑块指数有密切的关系,可以为指导临床治疗提供理论依据。

Objective To explore the comparison of UA,Lp-PLA2,D-D,Hcy and Fbg levels of acute stroke patients with carotid artery stable and unstable plaques.Methods Seventy-five acute stroke patients with carotid artery unstable plaques who were admitted and treated in The Sixth Hospital Affiliated to Guangzhou Medical University from July 2017 to February 2019 were selected as the observation group.Seventy-five acute stroke patients with carotid artery stable plaques in the same period were selected as the control group.The UA,Lp-PLA2,D-D,Hcy and Fbg expression levels of patients in two groups were observed.The relations between plaque index and UA,Lp-PLA2,D-D,Hcy and Fbg for acute stroke patients with carotid artery unstable plaques were observed.Multifactor regression analysis of plaque index of acute stroke patients with carotid artery unstable plaques were observed.Results The UA,Lp-PLA2,D-D,Hcy and Fbg expression levels of patients in the observation group were all higher than those in the control group.Plaque index and UA,Lp-PLA2,D-D,Hcy and Fbg for acute stroke patients with carotid artery unstable plaques showed obviously positive relations.Plaque index was taken as the dependent variable and UA,Lp-PLA2,D-D,Hcy and Fbg were taken as independent variables to establish the regression equation Y=7.675+0.058X1+0.086X2+0.092X3+0.027X4+0.048X5.The multiple stepwise regression equation had statistical significance(F=85.437,P=0.000).Conclusion The UA,Lp-PLA2,D-D,Hcy and Fbg expression levels of acute stroke patients with carotid artery unstable plaques have close relations with plaque index,which can provide theoretical basis for guiding clinical treatment.
