黄体酮阴道缓释凝胶用于体外受精-胚胎移植黄体支持的效果观察及护理

Effect of progesterone vaginal sustained-release gel for in vitro fertilization-embryo transfer luteal support

目的探讨黄体酮阴道缓释凝胶(雪诺同8%)用于体外受精-胚胎移植患者黄体支持的效果及护理要点。方法选择2017年1至12月在吉林大学第二医院生殖中心接受长方案促排卵治疗,行新鲜胚胎移植的200例患者作为研究对象,按照不同的黄体支持方案分为对照组和观察组,每组各100例。对照组肌内注射黄体酮针剂,观察组使用黄体酮阴道缓释凝胶。比较两组患者的临床妊娠率、活产率、自然流产率、生化妊娠率、不良反应率。结果观察组临床妊娠率、活产率显著高于对照组(P<0.05),不良反应率显著低于对照组(P<0.05)。两组自然流产率、生化妊娠率比较,差异无统计学意义(P>0.05)。结论黄体酮阴道缓释凝胶用于体外受精-胚胎移植患者黄体支持,能有效提高临床妊娠率和活产率,降低不良反应率。

Objective To investigate the effect of progesterone vaginal sustained-release gel(8%)on luteal support in vitro fertilization-embryo transfer patients.Methods From January to December 2017,200 patients who received long-term regimen of IVF-ET and fresh embryo transplantation were selected from the Center of Reproductive Medicine of the Second Hospital of Jilin University,and divided into control group and observation group with 100 cases in each group according to different luteal support schemes.The patients in control group were treated with progesterone by intramuscular injection.And the patients in observation group were treated with progesterone vagina slow-release gel.Clinical data of the patients in two groups were compared,including clinical pregnancy rate,live birth rate,spontaneous abortion rate,biochemical pregnancy rate and incidence of adverse reactions.Results The rates of clinical pregnancy and live birth in the observation group were significantly higher than those in the control group(P<0.05).And the incidence of adverse reactions was significantly lower than that in the control group(P<0.05).Conclusions Progesterone vaginal sustained-release gel can effectively improve clinical pregnancy rate and live birth rate of fertilization-embryo transfer patients in vitro as well as reduce the incidence of adverse reactions.
