阳和平喘颗粒治疗哮喘-慢性阻塞性肺疾病重叠肾虚痰浊证临床观察

Clinical Effect of Yanghe Pingchuan Granule in Treatment of Patients with Asthma-Chronic Obstructive Pulmonary Disease Overlap with Kidney Deficiency and Phlegm Turbidity

目的观察阳和平喘颗粒对哮喘-慢性阻塞性肺疾病重叠(asthma-chronic obstructive pulmonary disease overlap,ACO)肾虚痰浊证的临床疗效。方法将40例ACO肾虚痰浊证患者,采用随机数字表法分为对照组和治疗组,每组20例。对照组给予常规西医治疗,治疗组在常规西医治疗的基础上,加用阳和平喘颗粒,疗程为14 d。治疗前后分别检测两组患者血清白细胞介素-2(interleukin-2,IL-2)、IL-13、免疫球蛋白E(immunoglobulin E,IgE)水平,以及呼出气一氧化氮(fractional exhaled nitric oxide,FeNO)水平和肺功能。结果与治疗前比较,两组患者治疗后血清IL-2水平明显升高(P<0.05),血清IL-13、IgE水平和FeNO水平明显降低(P<0.05),第1秒用力呼气量(forced expiratory volume in one second,FEV1)、用力肺活量(forced vital capacity,FVC)、第1秒用力呼气量占用力肺活量百分比(forced vital capacity ratio in one second,FEV1/FVC)明显升高(P<0.05);治疗组治疗后IL-2、FEV1、FVC、FEV1/FVC升高值和IL-13、IgE降低值明显大于对照组(P<0.05)。结论阳和平喘颗粒联合西医常规疗法治疗ACO肾虚痰浊证,可减轻炎症程度,降低气道高反应,改善患者肺功能。

Objective To investigate the clinical effect of Yanghe Pingchuan Granule in the treatment of asthma-chronic obstructive pulmonary disease overlap(ACO)with kidney deficiency and phlegm turbidity.Methods A total of 40 patients with ACO with kidney deficiency and phlegm turbidity were divided into treatment group and control group using a random number table,with 20 patients in each group.The patients in the control group were given routine Western medicine treatment,and those in the treatment group were given Yanghe Pingchuan Granule in addition to the treatment in the control group.The course of treatment was 14 days for both groups.Serum levels of interleukin-2(IL-2),interleukin-13(IL-13),and immunoglobulin E(IgE),fractional exhaled nitric oxide(FeNO),and pulmonary function were measured before and after treatment.Results After treatment,both groups had a significant increase in serum IL-2(P<0.05),significant reductions in serum IL-13,serum IgE,and FeNO(P<0.05),and significant increases in forced expiratory volume in one second(FEV1),forced vital capacity(FVC),and FEV1/FVC ratio(P<0.05).Compared with the control group after treatment,the treatment group had significantly greater increases in IL-2,FEV1,FVC and FEV1/FVC ratio and significantly greater reductions in IL-13 and IgE(P<0.05).Conclusion Yanghe Pingchuan Granule combined with routine Western medicine treatment can alleviate inflammation,reduce airway hyperresponsiveness,and improve pulmonary function in the treatment of ACO with kidney deficiency and phlegm turbidity.
