温针灸联合手法推拿对气滞血瘀型腰痛患者症状疼痛评分及生活质量的影响

Effects of needle warming moxibustion combined with massage manipulation on symptoms, pain score and quality of life in patients with Qizhi Xueyu type of low back pain

目的:研究温针灸联合手法推拿治疗对气滞血瘀型腰痛患者中医症状、疼痛评分及生活质量的影响。方法:将70例气滞血瘀型腰痛患者采用随机数字表法分为观察组与对照组各35例,对照组予以镇痛、抗炎等对症治疗联合护理,并配合局部微波照射治疗,观察组患者在此基础上实施温针灸联合手法推拿治疗。比较两组患者治疗14 d后临床疗效,视觉模拟评分法(VAS)、日本骨科学会腰椎疾患综合评分(JOA)、Oswestry功能障碍指数(ODI)、腰椎活动度(ROM),神经传导速度变化,以及生活质量(QOL-100)量表评分变化。结果:治疗14 d后,观察组患者治疗总有效率明显高于对照组(P<0.05)。两组患者VAS评分均较治疗前明显降低(P均<0.05),且观察组评分显著低于对照组(P<0.05)。两组患者JOA评分均较治疗前明显升高,且观察组显著高于对照组,ODI评分、ROM评分均较治疗前降低,且观察组低于对照组(P均<0.05)。两组患者腓总神经、腓浅神经传导速度均较治疗前明显提高,且观察组显著高于对照组(P均<0.05)。两组患者生理、心理、社会关系、生活状况、独立能力五个方面评分均较治疗前提高,且观察组高于对照组(P均<0.05)。结论:对气滞血瘀型腰痛患者予以温针灸联合手法推拿治疗,疗效确切,可有助于疼痛缓解与症状改善,利于患者恢复。

Objective:To study the effects of needle warming moxibustion combined with massage manipulation on TCM symptoms,pain score and quality of life in patients with Qizhi Xueyu type of low back pain. Methods:70 patients with Qizhi Xueyu type of low back pain in our hospital were randomly divided into observation group and control group according to the random number table method,with 35 cases in each group. Control group was given symptomatic treatment combined nursing of analgesia and anti-inflammation and combined with local microwave irradiation,and observation group was given needle warming moxibustion combined with massage manipulation on this basis. The clinical efficacy after 14 d of treatment,and visual analogue scale(VAS),waist and leg pain symptom score(Japanese Orthopaedic Association,JOA),Oswestry dysfunction index(ODI),lumbar range of motion(ROM),nerve conduction velocities and quality of life(QOL-100) scale before treatment and after 14 d of treatment were compared between the two groups. Results:After 14 d of treatment,the total effective rate of treatment in observation group was significantly higher than that in control group(P<0.05). The VAS scores in the two groups were significantly lower than those before treatment(all P<0.05),and the score in observation group was significantly lower than that in control group(P<0.05). The JOA scores in the two groups were significantly higher than those before treatment,and the score in observation group was significantly higher than that in control group. The ODI score and ROM score were lower than those before treatment,and the scores in observation group were lower than those in control group(P<0.05). The motor nerve conduction velocity and sensory nerve conduction velocity in the two groups were significantly higher than those before treatment,and the indexes in observation group were significantly higher than those in control group(P<0.05). The scores of five aspects of physiology,psychology,so-cial relations,living conditions and independent ability in the two groups were higher than those before treatment,and the scores in observation group were higher than those in control group(P<0.05). Conclusion:Needle warming moxibustion combined with massage manipulation has exact efficacy in the treatment of patients with Qizhi Xueyu type of low back pain,and it can help pain relief and symptom improvement,and it is conducive to patient recovery.
